I think you’re pushing sx here. I am a 61 year old retired Marine (jumping/rappelling out of helos) and a truck driver (3-12 hour shifts per week, loading and unloading 3 to 600 pound surgical cases every Friday, Saturday, and Sunday. I also am 40lbs overweight, have triglycerides levels over 500, HBP, and Morton’s Neuroma in both feet. I also drink too much on my days off and am nowhere near the 180lb “fighting machine” I was coming off Parris Island boot camp in 1986 (33 years ago) I think I will try a few things before I let someone cut my nerve! 🫡
Good morning Sir, My name is Dr. Jay Shah, an Orthopedic Hand Surgeon from Mumbai, India. I’ve been experiencing chronic left inguino-scrotal pain for the past 7 months. Despite my family's strong medical background-my father and grandfather are both MDs in medicine, and my eldest brother is a Hepato-Biliary Oncosurgeon in Mumbai-this case appears to be quite rare. None of the doctors we've consulted have come across something similar. As someone who has followed your work closely, especially due to my interest in hand surgery, I’ve noticed that you’ve managed several cases with similar presentations. I believe your expertise could shed light on the best approach for my treatment. Recent MRI neurography reveals the following: - Asymmetric thickening and increased signal of the left ilioinguinal and iliohypogastric nerves, as they course between the internal oblique and transversus abdominis muscles in the lower anterior abdominal wall and into the inguinal region, with the ilioinguinal nerve entering the spermatic cord. - Mild asymmetric thickening and increased signal of the left spermatic cord. - Maintained contiguity of the nerve fibers, without any nodular thickening, mass, or cyst seen in the canal of Nuck. - No overt perineural fibrosis is observed. - Subtle thickening and increased signal of the left inguinal ligament. - Surrounding muscles and adjacent vascular structures appear unremarkable. I’ve been in significant pain, which is now impacting my daily activities. Unfortunately, oral prednisone and pregabalin have not provided any notable relief. Could you kindly advise on the best course of treatment moving forward? I look forward to your response. Thank you. Sincere regards, Dr. Jay Shah
I'm experiencing nerve pain from left inguinal hernia surgery right now. It's brutal. A car ride feels like knives plunging into my testicles. Luckily, I have great health coverage and rather than going in circles I know exactly what it is. I will request surgery with a nerve pain specialist.

Hi Dr Tim, I'm a graphic designer & video editor. I wanted to work with you to re-create your RU-vid thumbnails to increase the visual appeal of your channel. We could discuss this in detail if you are interested, thank you.
Hi Doc. I burned my feet in hot water/mag surfate salt bath. I have pain and burning mostly when I’m walking or standing. Although this was a first degree pain, I’m wondering what has happened and how can I reverse it.
I injured my back lifting heavy tiles improperly 11 months ago. The pain is on my right side, radiating to my right hip, lateral and anterior right thigh. The MRI shows disc buldges and disc protrution at L2, L3, L4, L5, facet hypertrophy and ligamentum hypertrophy throughout lumbar. I get relief from laying in bed and with Mckenzie extentions sometimes, but sitting and standing too long at work brings it all back. My symptoms seem like meralgia parastetica with low back pain. What can I do? It is destroying my life.
Why won’t you take insurance? Other doctors performing this operation take insurance and the patient pays the difference . Why can’t you do this so people can get help? Whatever insurance pays you accept but patient agrees to pay the rest. Win win. All you have to do is opt in with insurance and explain this to your patient .
Im 66 from SA nd im selfemployed since now i cannt do anything my feet legs knees give me sleepness night i tried everything they told but nothing happened especialy this wake up is killing me nd i dont have treatment
Mine is constant and can be debilitating, hard to walk, work and depressing. I had a recent radical robotic Laparoscopic nephrectomy and for 2 weeks the pain was gone just numbness in upper groin and right leg. Now pain is back and though I'm in still recovery I am finding it very depressing and painful.
@@varunsharma6217 hi thanks for the comment. I am recovering slowly from nephrectomy surgery. Still sore in places with movement but being careful before going back to work as it's very physical. With regards to the groin pain, that is still here. I've had it before the nephrectomy so I don't think related to cancer. But I went over ultrasound result and except for a bilateral epididymis cysts & atrophy of right testicle there is no abnormality. The pain is only in right side. I have been looking through medical history. When I was around 2 I did have surgery for a undescended right testicle. But I don't know why I'm getting these pains can't be "blue ball" I get this pain even with no excitement. I can get pains from walking or even just standing for long periods. I'm seeing urologist surgeon in 2 weeks so I'm going to consult with him. I can't live on Advil for rest of my days.
Annular tears are horribly under diagnosed and may not show on mri,,this will can flyid leakage on nerve roots even without compression destroying your life,,a discogram will diagnose this
So glad to hear you’re doing so well. I am in the same place you were and am contemplating the same surgery. I Would be extremely grateful if you would share how long your recovery took. Thanks
I started zinc 14 mg a day by day 5 my foot tingling, numbness in toes and pain pressure point slowly began to subside currently at 10 days symptoms have decreased from a level 5 to a 2. Can’t say for sure it was the zinc but that has been the only added supplement or change to my diet and intake in the last month.
I wish I was in Nevada, I'm in Canada and I'm feeling unsure if the specialist truly knows what to do. I was diagnosed almost a year ago, and have been given nerve ablations but they fail quickly. I believe surgery is my only solution
Is there any relief for those with (heredity) Sensory/Motor Axonal Polyneuropathy? I've already had decompression surgery L-3-5 for severe spinal stenosis.
Hi I'm in the UK. I've just had the same surgery from a brilliant man in Manchester. It's an extremely painful condition. I got it from 35 years plastering and bending and twisting . Good look with you recovery
Wow! I had a concussion and have been constantly dizzy off balance and in pain i need this done!! Im in the uk though is there anyone i can see that you recommended closer to me? Thank you
Can this help with dizziness related to migraine?? I had a concussion 11 years ago i can barelt function from the pain and dizziness 😢 nothing is helping
I can’t believe you have it so “right on”. You are speaking right to my situation throughout the peripheral neuropathy video. I sure wish I had watched it live. Better yet I wish I could have a discussion with you concerning my situation. Just a little history, no I changed my mind. I forgot this is viewed by the public. I have diabetes, diagnosed first as type two, then rediagnosed years later as type one. The neuropathy came later. I’m sorry this is too much information for you to have to read and for the public to see. How can I get in touch with you? I have seen endocrinologist, neurologist ,cardiologist almost every kind of ‘olgist ‘ practicing. And no one is getting it right. Meanwhile, the condition is probably worsening and I am very very uncomfortable. I can see & feel the situation worsening. It’s almost like they don’t hear me. But you have heard somebody because you’ve pointed it out throughout this lecture. I could give you hundreds of reasons why I should not give up. However It seems hopeless since Im not diagnosed correctly consequently not treated. I know this because I’m not getting the evaluation or a recognition and/or explanation of my clearly described symptoms. After listening to you, I’m certain . I need some help. I live in Hampton Virginia. I will come to you if possible unless you have another idea,a colleague perhaps but not preferred. I know one thing : we can’t diagnose, treat, follow and evaluate through a podcast. I’m stuck please unstick me. I can I think describe to you everything that is manifesting as signs & symptoms. . You ,sir are brilliant. And I believe you can help me. But I salivate over maybe a bone you might throw. Wow it’s 4 am . Thanks for reading this if you are ,if you’re not and someone is, please pass this along. I’m serious about needing some help. I’m stuck here. My email is RRILEY 24@cox.net THANKS , rr P.s. I have a tremendous amount of pain in my right, but it feels like it’s in the bottom of the bone. I can’t sit on it for any length of time. That’s not all but that’s a big one. I can’t sit for more than one or two minutes. Also, my son and I are coming to Las Vegas in September to see Shania Twain, but I’ll be able to see you then. Be nice to be able to talk.
I am a 68 yr old female, thought I had Sarcopenia until I found a video on this subject. Now I know exactly what the problem is, I can treat it! Seen numerous doctors/physios, about my hip and leg pain, not one of them has suggested that. I may have Meralgia Parasthetica, just keep given me painkillers.
I can't find any literature on Piriformis Syndrone caused by Bulging Disc. The Bulging Disc is not healing. I've tried the chiropractor, the injections and the Physical Therapy. I can't have surgery on the Piriformis if the cause is the bulging disc. My doctor doesn't recommend surgery, but the pain doesn't go away. The more I stretch or exercise, the worst it gets.
I have been suffering with this for about 20 years. Meds have stopped helping. I had 2 nerve blocks. 1 alleviated the pain completely for about 6 months. The second helped only slightly. Do you know of any doctors in the NYC area who can disconnect the nerve?
Thank you 🙏 After suffering horribly for the last year & a half, finally getting dx’d a few months ago & enduring abdominal injections monthly that are becoming less & less effective - there may be hope for me yet! I want my body (and my life!) back!!! 🙏❤️
Had a very long back surgery in 2005 since then left thigh is numb, with burning what I call shock like feelings. some days much worse than others. What’s the odds 20 years later a decompression would fix it. I could live with numbness. The pain gets brutal at times. No one knew what was wrong with me until I came across a you tube video by chance once day!!
You are the first person I've ever heard of to suffer the same way I suffered for 23 years. Not sitting and thus not being able to work etc, etc, and etc. /you know. Mine began when a surgeon left a surgical sponge in me for almost a years. After many years they decided I had pudendal nerve entrapment, among other things. I hope you find relief and lasting health.

About 80% of what you shared I'm going to what's scheduled for epidural was the epidural be a part of like a surgical they tell me I don't have diabetes I have high blood pressure and done the EMTs EKGs quite a bit of stuff but to no avail I'll be seeing the doctor today I'm just confused but my feet are swollen the blood on my ankles first it was one right now or nothing on the other one what to do
Dr Tollestrup, I get the feeling that you wouldn’t have ever done decompression surgery on me. What should the Doc who did my decompression surgery have done? I don’t think he did any exam. By the time I met him I was on the operating table. He was going to cut and he didn’t care about an exam because he didn’t to my knowledge do one. I got a Cortisone shot in each foot 4 1/2 years ago and have not walked one step since that day. I was in such tremendous pain after that. After that the podiatrist told me to go to a neurologist because he thought I might have peripheral neuropathy. I do.For years and years I was told I had metatarsalgia. As I got into more and more pain and even bought a scooter chair to use a lot of the time because of the pain doctors just told me I had metatarsalgia and needed better shoe inserts, or drugs. I am thin and don’t have diabetes and it never crossed my mind that I could have neuropathy.I had the decompression surgery and it did nothing except leave scar tissue. I had never met the surgeon except via telemedicine. I’d sent my MRI, ultra sound, and sent him the records Nobody’s given me any insight at all. So I went to another city where he did the operation and he never even saw the Morton’s neuroma when he opened up my foot but he went ahead and did the surgery anyway. The decompression surgery. When I told him months later it hadn’t worked (oh and by the way his office never called me to see how I was), he said don’t ever get any more surgery again because that didn’t work. He was completely not empathetic. Really bad experience. It took a long time to recover because I was in so much pain. Thank you so much for your informative video.
