While the main line did die out with the childless Anna Maria, you're not entirely correct. A few cadet branches have sprung from the main line over the centuries and some of those still exist today. If not Habsburg influence at the time Gian Gastone died, Tuscuny would have passed to one of these cadet branches, but instead they secured it for one of their relatives whom had a far weaker claim on the grand duchy compared to the Medici's of Ottaiano. So apart from the Neapolitan princes of Ottaiano, I believe there was also a branch based in Verona that still exists.
