Hey Doc, 5 months ago I took Mobic 15mg, for two days and I had really cloudy consciousness and dizziness. I knew the risks for tinnitus, stomach ulcers etc, how is it possible that this happened? Most of my bloodwork came back fine except for elevated liver enzymes, high cholesterol and low testosterone. Doctors say it wasn’t the Mobic, but I took it several times over 2 months and made me very dizzy the first time, the second time and the third time I felt spaced out for 2 months. I just want to know what happened to me?
1:33 so true! People think my son is being energetic when he’s actually quite lethargic compared to his standard operating mode. It’s hard to convince people that haven’t met him before that he’s normal a spit fire so if he’s seated for even a couple of minutes, the wheels have probably fallen off.

I was overdosed by a GP with no pharmacist I was seriously like a vomit commit he told me it was anxiety-Jesus Christ this and flipping Haliperidol then I developed Neuroleptic Malignant Syndrome..I am still on this but I changed no more (plane Carbamazepine slow release )I was in a coma from this -It doesn’t have to be a PX overdose Dr’s screw up if they ignore it -And don’t have a Pharmacist you can land up seriously damaged for life I changed my GP now weekly blood tests
I took 12,500mg of this in a SI, serum peaked at 460, and then Ammonia levels at 164. I was given Narcan .4 twice to stop the encephalopathy and then Lactulose for the elimination of ammonia. Two days in the ICU, two days in the Step down unit then was discharged…almost got intubated and dialysed but it wasn’t necessary.

Sorry not something I know much about. Most people I have treated have recovered well if they make it thorough the initial few days, the early management is important.
i love how inaccurate this theory of single dosing is when compared to what i know to be functionally true. in regards to toxicity my theory is if you split up a single dose into 3-5, make the first and last smallest, biggest in the middle, you allow your body to more effectively conduct biotransformation on the toxins produced. This bell curve approach to dosing is not understood by science for some reason. Also what supplements actually work is not understood by science either. People always suggest 5-htp supplements and for me it does nothing . What works for me is actually quite simple and cheaper: Boost nutritional energy drinks, emergen-c, sushi. The 3 listed forms of nutrition were life changing for me when recovering. Although i no longer dose like I use to, my recovery habits have carried over to my regular routine. Hopefully someday the science can prove my theories and explain why my routine works so well.

Very true but the active compound is still the cocaine alkaloid so I disagree that it's very different. It would have been much weaker than pure coke though!
I have been taking Sodium Valproate (2 x 200mg daily) since early 2019. It’s used for my Emerging Mood Disorder and Anxiety Disorder with panic attacks. Am I at low risk with the current dosage?

A 24 y/o male presents to the ER with tachycardia. They have ingested 900mg wellbutrine XL (150mg was prescribed; maximum FDA approved dose is 450mg), 60mg Escitalopram (20mg prescribed and maximum dose), and aripiprazole 10mg as prescribed. All of this 10 hours before taking 84mg Esketamine intranasal (as prescribed, and maximum dose) The patient complains of fast, strong heartbeats. There is no shortness of breath or pain in the chest. Heart rate is stuck at 140-150 bpm and is stuck at that rate for an hour before going to the ER. How would you manage this case, and what's the diagnosis?
Interesting case, certainly very strange to have that presentation 10 hours after ingestion. Ketamine doesn't usually cause such tachycardia. I can't make diagnoses over the Internet but the approach would be to do a full history and examination and most importantly an ecg, which should give a clue as to the origin of the tachycardia. What was the outcome of your case?
While I was on Sodium Valproate I went into Septic Shock, a few days later my psychiatrist took me off them, and I have not had any shock symptoms. That was 13 years ago
That makes sense. I just looked it up and that's a rare effect of valproate but there are some cases like yours reported in journal articles going back to the 1980s!

They used coca. The leaves first chewing. They turned it into powder by a German scientist to get it across the Atlantic ocean because the leaves weren't super stable
For most healthy adults it's safe to take 8 tablets of 500mg in 24 hours but this might vary based on your personal information and I can't give medical advice here.
