I'm a nobody, but I would think it would be illegal for her not to fill the paperwork out. What information she puts on there may be another issue. I don't know but good luck.

I suffered a massive saddle pulmonary embolism,(nearly costing me my life)after being laid up for 2 months following heel surgery...... the doctor did absolutely nothing to prevent this from happening, blood thinners, compression socks,.. nothing.Never even mentioned the risk(I was 59 5'8"" 250 lbs )....I now have a malpractice suit pending working with my malpractice attorney

Something is happening to my left knee replacement...the latest I was able to find online for myself is possibly peroneal nerve damage...is it at all I can speak with you about my experience...I'm desperate for help that I'm not getting.

I suspect this might have happened to me but how was it determine that it was due to how he was positioned during surgery? There were no cameras in the OR nor was my surgery recorded.

Yes, I wouldn't be disabled if I had competent Dr. I will probably never be able to go back into construction. 44 now with MRSA and Impitigo on top of mental issues worsened from this. I can't exert energy to a sweet or I'll get more infections. I've represented myself before to a Jury and did quite well. Deliberated with the Judge and DA and they both verbally expressed their respect at the end of the trial. Considering law school myself now, but lack of funds. I got screwed out of a lot of SSI back pay I was planning to use for schooling due to the doctors failure to diagnose. I'll have to figure something else out now, I don't want to be on SSI forever.

Yes I agree, failure to timely diagnose. I'm doing research for my own case. My former Dr would not diagnose skin infections I was having on my legs. Three visits in a row she blamed me for not taking better care of myself as I have been homeless. I still showered three time a week. Three visits in a row she said if it continues to be a problem on the next visit she would refer me to dermatology for testing, yet she did not. Over a year of her not fulfilling her responsibility and blatantly contradicting herself by not referring me to dermatology and continually blaming me I'm thinking counts as (Gross Negligence). After being pressured by an E.R. Dr having to go to the E.R. several times my Dr. finally sent the referral for dermatology. By this time the infections had gotten worse and bigger and more painful on my legs. Dermatology tested an infection and it was positive for MRSA. Nov 1 2019 is when the infections started. I am now on my second dermatologist and the infections have been spreading everywhere up to my face, but the bulk of my problems are now on my testicles. Far from pleasant. The second dermatologist diagnosed me with impetigo as MRSA can mutate into impetigo apparently. Prolonged impetigo can lead to kidney failure so I now have to get tested for kidney problems. Dec. 2019 I filed a SSD claim. It was granted just this week SSI, but back pay to only Jan 2022 as failure to timely diagnose led to a loss of two years back pay, approximately $25,000. I intend to sue for the failure to diagnose as it has led to more complicated consistent painful problems, inability to function / work = lost wages lost disability pay ecetera. Several years ago I lost a finger tip to a construction accident which would explain the contraction of MRSA.

What I don’t understand about malpractice is consent. I had a procedure done at 7-8 years of age that never had to be done in the first place. It has left me with permanent scaring (keloid, which is quite ugly). Now who’s responsibility is it? I would say both my health insurance company and my pdoc.
