I have crooked teeth in front and a bit of protruding jaw , i consulted 2 doctors both advised for all 4 wisdom teeth removal along with invisalign. However i don't want to extract my teeth. Is there any alternative way for the same?
Hi! Do they offer expanders for the lower jaw as well or is this just a palate expander. And if so, how would you go about match expansion on the lower jaw.
Great question. The hard skeletal palate has 2 bones that are connected with a suture that closes around 12-15 years old so expansion of the upper will fill in new bone. This means you can widen the upper jaw. The lower jaw's suture closes <1 year old so there's no such thing as a lower jaw expander. The lower jaw dictates how much you can widen since it's a relatively fixed width. Braces or Aligners can widen the teeth which effectively can mimic low jaw expansion though. So if you expand the upper, then you'll need braces or aligners to match the teeth width together.
so it doesn't push the top front teeth out to the front cuz thats my problem, I need the front 4 teeth pushed more forward I have an underbite. is there anything fir this ?
Good question. The expander WIDENS the posterior arch (back teeth) --> No effect on front teeth. It sounds like you need Invisalign Treatment (not Invisalign Expansion). Go see an orthodontist for an evaluation. Thanks!
Wish IPR was a thing when I had braces at kid :) Is it possible to get invisalign where they don't go all the way down to your gums, nor inbetween teeth at transition at gums, say they would cover 80-90% of the teeth but not to the actual gum? I've had invisalign retainers made, unfortunately they all touch my gums and I had to do extensive nail clipping everywhere on them. They made my gums completely white when wearing it before I cut off all the plastic overlapping the gums- the invisalign lady who did the scan didn't care and was of no help unfortunately. I've seen some retainers post braces/invis be 50% of the teeth. Cheers
Great question. Invisalign retainers are designed to be trimmed before touching your gums so there's no covering of gum tissue. It sounds like you got a generic clear retainer and not an Invisalign branded clear retainer. There's a difference in materials and manufacturing between Invisalign Retainer vs Generic Retainer. Most generic retainers I see cover the gums as well, which sounds like you received. Go find someone that uses Invisalign retainers (not the cheaper alternative).

Great question. If your arch width is smaller than a normal orthodontic wire, then yes. If it is the same size or bigger, then no. However, you will get a nice curve of the smile and that usually helps create the illusion of a broader smile so the orthodontic treatment will give you an overall better smile. Hi Dr great tips. Question how do I heal the white cheek marks I get when I punch my cheek when inserting liners? Will the cheek marks go away when I finish treatment? I’m on week 28 of my trays. Any advice on how to heal the white cheek lines or prevent them?
Hi Chandra, I have pinched my cheeks when I’m putting my tray in. To avoid this, I place a finger between my cheeks and the tray while I’m placing them in. It has really cut down on those painful pinching😬
Interesting question ... I'm not aware of my patients getting a cheek irritation when inserting the Invisalign trays. It sounds like you need to use another finger to pull your cheeks off your teeth to avoid this. If the question is about the white horizontal line on the buccal cheeks that is adjacent to your teeth's biting surface, then that's called the 'linea alba' and caused by the friction/biting of your teeth. It's very common and sometimes goes away on it's own when the bite improves. Hope that helps!!!
I routinely provide Invisalign treatment for patients that are missing molars. However, if you are looking to move those molars, then sometimes braces are better. I recommend an orthodontist consultation. Good luck!
@@drpassamano I already had orthodontic treatment with braces. I had braces for 5 years and now regularly wearing retainers. However, I have noticed that my teeth are relapsing. I probably think Invisalign could help. Also, I don’t plan moving my molars

Yes, this patient did have a lot or pre-existing dental issues like recession on multiple teeth including that Max left incisor. That's what a decade of neglect will do. We turned him around and he's on a path to maintain his awesome new smile though.
Thanks for the video. Does the Invisalign palate expander effect speech like the metal one can? I’ve been recommended this for my 6 year old who is worried about speech impact.
I think the Invisalign Palatal Expander has more speech issues than the traditional metal one. The Invisalign one is bulkier and covers most of the area where the tongue creates sounds. The metal one has less coverage so less speech issues. I use both Invisalign Expander and Slim Metal Expanders and I'm still doing much more regular ones. Sometimes new isn't better.
Great question. The mandibular symphysis closes by 1 year old while the maxillary suture closes around 11-15 years old so there's no such thing as skeletal expansion on the mandible. HOWEVER, the lower teeth can be tipped (dental tipping) to give the appearance of a wider jaw and that's exactly what's done. It's best to do orthodontic treatment around 12-16 years old to avoid gum recession issues that occur when big orthodontic changes are done as an adult.

How long it take you to fix the last one i think i have similar issues with my lower teeth. Im 20 and i dont wanna have braces😅. Can we fix it with ipr+invisiline?
The last example was 20 months. It's slightly longer because I was trying to do lip retraction which is more difficult with Invisalign than Braces/extraction. Hope that helps!
Good question. The tooth soreness is dependent on pressure on teeth, so it will depend if your doctor programmed movement on some teeth or has delayed movement. It's very case specific, but I'm sure you will feel something soon. Good luck!

There’s nothing that just works on those teeth without having to mess with all the other teeth it is what I’m asking I suppose .?
Absolutely correct. Processed foods mean we aren't chewing as much as we used to (2-3 hours a day) and jaws aren't be developed. As with anything in modern life, there's a fix for that and its jaw expanders or we can go back to chewing foods for 2-3 hours a day (sarcasm). Great question. There's no difference in Damon Braces vs Regular Braces (many studies have already dubunked Damon Braces' claim of less extraction). I suggest you see an orthodontist and have a smile simulation to see what your result would look like. There are options to reduce flaring that include Extraction, Torque Control, IPR, TADs, etc ... Choosing a good orthodontist is the most important thing you can do.
@@aac2289 Great news! That correct that sometimes extraction can retract the lips too much. Sometimes extraction can get ideal lip position so it's all case specific and it sounds like you can achieve an awesome smile without extraction. Good luck!
@@aac2289 If you are asking about trusting your doctor, then it sounds like you need to get a second or third opinion. The patient should be confident in the treatment plan prior to starting treatment. Trust your doctor is #1.
Palatal expanders (without surgery or TADs) are generally best treated around 7-10 years old and can be used with somewhat effectiveness in early teens. Once the suture is sealed (around 12-15 years old), then sutural expansion is not predictable.
There's a lot of factors (age, bone quality and quantity, position of teeth, gum tissue level/recession, etc) that will determine if you are even a candidate for palatal expansion. If you are not, then IPR is safe and effective for reducing flaring. I would suggest a consultation with an orthodontist.
I have flared teeth too (both upper and lower teeth are flared) and my orthodontist said that it can be fixed by wearing rubber bands only. I did a bit of a research but all I found was either extractions or IPR. Do you think that it's possible to fix flared teeth with rubber bands too? I'm kind of getting a bit skeptical. Great question. Your orthodontist is not wrong, but the amount of angulation change with rubber bands is extremely small ... maybe 1 or 2 degrees at most. It's probably not enough to change most people's flaring issues, but it could help a little. It sounds like you have braces, I have been successful with Handfile IPR (not using the disk style) and sling powerchain below the bracket where the powerchain is about 2-3mm down from the braces on the side of the edges of teeth (not gum side). That's what I do when I'm looking to get 4-5 degrees with braces. Good luck!
Hi Doc, I'm nearly done with Invisalign and wore braces 20 years prior. My orthodontist is happy with my result but I feel my teeth still very much flare/stick out. I was told I can't get a better result than this and that teeth are meant to flare out a bit. However, that's contrary to the examples I'm seeing that you have posted and I recall a better result when I wore braces 20 years prior (but I was told that was my younger/smaller teeth). Tough question when the orthodontist says you're done, but you're actually not done. If you received an Invisalign Complete package, then you can order more trays without a cost to the orthodontist. If you have one of those limited plans, then every order costs the orthodontist. All of these cases are with the Complete plan in this video. Regardless, have a conversation with the orthodontist that you'd like to reduce the flaring another 3 degrees and you saw a video about IPR and wanted to try that out. If the doc says no, then ask about offering to pay the lab fee. Give that a try!
I appreciate you wanting to see me!!! There are plenty of very good Invisalign orthodontists all over this World especially in the UK. It might take a little time researching and a few consultations, but I'm sure you will find someone nearby.
Great question. Non-extraction IPR for teeth re-angulation has very little or no effect on the lips because the roots are not translated, but tipped inward. Extraction treatment with braces will have a noticeable change in the lip position. I tell patients that IPR only will maintain the lip position, so that's the big difference between extraction vs non-extraction IPR.
I get that comment sometimes when patients are not removing the trays correctly. If you are pulling UP / DOWN (vertical), then it will feel like you are pulling your teeth out of your face. I do teach my patients to do a lateral rolling motion so the trays can disengage from the attachments for less pressure on the teeth.
I want my flared teeth fixed so bad 😢, It’s ruining my confidence and I don’t want it to worsen. I am 19 and my family has been experiencing financial problems, they’re experiencing lots of stress and they tell me that there’s no money for getting braces. I wanna work or start my own business but still, I don’t have money to start it and I can’t risk stopping school to work. Still, I’m gonna try to find some opportunity to work my way up so that i can afford getting treatment for myself, maybe even help my little brother so that he won’t experience the same issue.
Sending good vibes your way. You are still much younger than all of the examples in this video. These patients are 23, 26, 34, and 46 years old. You have plenty of time. Just remember that braces and cars used to cost the same price. Braces has never been more affordable.
I never had an open bite until after traditional braces. My orthodontist removed my braces before they should’ve come off, unfortunately. It was during the pandemic and I think he was trying to get all cases wrapped up, and now I live two states away so there’s nothing I can do about it with that ortho office. It’s not 6mm but it is noticeable enough and I feel like from the side view you can really see my teeth pointing out at an angle. I recently saw a dentist for an exam and he mentioned Invisalign could help, so here I am scouring the internet for successful cases before I sink more money into my smile. Hello doctor i have diastama on my front teethand a gap between 12 and 13 and 22 and 23 And lower the lower there is a gap of a tooth between 42 and 43 its a natural gap i used to have a Maryland bridge but i took it put causing somuch pain because of pressure and i have a tight contact at the rest of my teeth feeles little bit of mobility may be 1 ... is there any options i can do ... either braces or clear tray do you think am i eligible for either of these...
It sounds like a visit to the orthodontist would be great for you! Many of these consultations are complimentary (for the first visit). Maybe visit 2-3 different doctors to see who you like, and which can solve your issue the best. Good luck!
Great question! It depends on where teeth are to be moved. Sometimes braces is faster and sometimes Invisalign is faster. Check out Custom Braces instead of Damon braces. If you're looking for speed with braces, Custom Braces is better and faster than Damon braces.
Do you measure for transverse measurement before/after? I'm looking at Invisalign. I have a great smile but lots of muscle issues. New orthodontist thinks widening my smile will create more room for my tongue and thus less muscle tension and pain. I have a 32mm transverse measurement currently and he's going to aim for 37-38mm with the Invisalign.
Great question! 32mm is on the more constricted for upper arch size. I like 36+mm for an arch size. It sounds like your Orthodontist knows what they are doing! I do find that patients get TMJ / Muscles soreness often when the upper and lower jaw sizes do not match up. So if you lower is wider than your upper arch, then widening the upper arch makes a lot of sense. Go for it!!!

hey doctor weird question but when some orthodontists say powerchains move teeth -1mm retraction per month do they mean the teeth going literally backwards in the bone 1mm? (talking about front 4s)
Not a weird question at all. In your example of premolar extraction, the size of the premolars is generally 7mm. I usually average about 0.75mm per month (for teenager) of retraction so it usually takes me 9-10 months to close spaces for a teenager and about 12-14 months for an adult. Second question, YES ... it means the entire tooth is being retracted. It's probably less than 1mm because of reciprocal forces, but in terms of simplicity ... 1mm is a commonly used measurement term but it's really more like 0.6-0.8mm/mo.

I have a question: Im from Bangladesh and here for treatment of bimaxillary protrusion, almost every orthodontist extracts 4 premolars. but one of the orthodontist said he can fix my class 1 malocclusion only with clear alligners without ipr or any other way to expand my arch. Is it possible to do so? or is he trying to scam me??
Interesting! I would go with an orthodontist that you trust with a proven track record. You will find that some doctors will push Invisalign because they are being pushed by Corporations (Invisalign). Make sure you find an honest and ethical doctor. Continue on your quest to get another opinion because the more educated you are, the better decision you will make for your treatment. Good luck!!!!!
Really good question ... You've probably read that the best ages to do expansion are in kids as young as 7 years old up to 12-14 years old. I have seen expansion work in 20-30 year olds with gaps opening between the 2 front teeth which means sutural expansion. If I was an adult, I would try the expansion and see what happens meaning it's non-surgical; no operating room needed. If it doesn't work out, then you will need to reverse and wear the trays backwards to get back to where you were. Make sure you are being monitored by an orthodontist. Teeth can be moved in 1 direction and back in the other direction, but it needs to be done carefully. If you're willing to take a chance, go for it! If you don't want to take a chance, then SARPE is the tried and true method, but that's surgery. Good luck!

@@jnorris32 Great question. Sutural expansion vs dental expansion is the difference between working and 'if it doesn't work out'. You need to have your orthodontist monitor you to see which way it's going (skeletal or dental). The patient will likely not be able to figure this out on their own.
Invisalign IPR for re-angulation of about 8 degrees usually takes me about 10 months without crowding and 14-16 months with crowding (because you have to resolve the crowding first).
My daughter just got her expander in on the top jaw. It's a permanent one and we go back in 6 weeks for the dentist to expand it. She got it last Wednesday. It's hurting her not in severe way but she doesn't like it. She's 8. Since we can't go to the dentist over the weekend I am curious if this new expander can be an option for her?
Great question! It sounds like she has a non-turning expander without a screw. There are some expanders that the orthodontist can activate 1 or 2 times and it expands without a key. There are pros and cons of these types of expanders though. I personally like to be in control with the amount of expansion and I do virtual expander checkups every 2 weeks. As far as the Invisalign expander goes, it does sound like it would work great for your 8 year old. The Invisalign expander is a new treatment appliance so not every dental professional is ready to use it for their patients. Wishing your 8 year old all the best on her orthodontic treatment!
The last case was the slight retraction of lips, improvement in chin profile and re-angulation of lower teeth (flared to upright). This was done with Invisalign and took about 18 months to accomplish. The current 2024 pricing on this is around $7k.
