Went through the same thing helping a buddy of mine load a BBQ into a van. Made a bad move and tore my tendon on my right arm (strong arm) at the elbow. My orthopedic surgeon advised me against doing the surgery due to the following reasons. I'm 63 yrs old and retired, I would lose maybe 20 to 30% of supination strength, the risks of complications are quite high (20 to 30%) and maybe higher due to my age. Other than the physical appearance of the bicep which is a bit bothersome, especially when you train and take pride of the way you look, I decided not to do the surgery. It's certainly not worth the risks.

My bicep is odd looking also, but it's only when I flex. I understand what you are saying about your age but there are conflicting messages between surgeons on this. Read the case studies I have on this channel. Ultimately, the case studies were a big part why I choose not to have the surgery.
