Was around Agent Orange in Vietnam, developed prostate cancer but thankfully my doctor caught it early and had radiation treatment, now cancer free. Nasty stuff, ironically I'm from New Jersey and some of the stuff was made in Newark, NJ on the banks of the Passaic River. I remember seeing hundreds of 55 gallon drums and wondered what kind on nasty stuff was in them.

As intimate an issue, prostate doc’s have a somewhat isolated or removed contact with prostate patients. They are never in the bedroom, bathroom, or in the department store during urgency. While some are overwhelmingly qualified, others can really only perform a DRE. The institutional priorities sometimes do not align with a man priorities; thus, he will spend a great amount of time researching and searching between the contradictions of each treatment, study, and clinical trial. Often, he will only find himself right back where he started. Another 30 minute appointment to not be heard.

2012 diagnosed and treated, 12 years later still dealing with all the damage. I had HIFU, one treatment and done. Don’t I wish, unfortunately I got it when I had it done it was still in trials. Over a 20 year career 16 deployments 4 to the middle east. The first thing research, don’t just accept what the first doctor tells you. Every doctor is going to recommend what they specialize in. And none of them tell you everything, like if you get radiation you have double the odds of a secondary cancer (bladder/colon). All treatments have their downside. And there is no one size fits all answer. My experience. Diagnosed So with the newer attitude of not treating early detection PC, how do you reconcile the odd patient the has a faster then “normal” advancing cancer. My first MRI showed one lesion “the size of a dime”, leading to a positive biopsy. But after being referred for a treatment and an MRI guided biopsy which showed two lesions “one the size of a nickel, and one the size of a quarter”, after only six months. And the doctors said “that shouldn’t happen, you don’t have small cell carcinoma”. Had I done nothing but active surveillance I might not be here. So there’s no set right answer, the whole process sucks and sometimes it may look like there’s no right answer. All you can do is weigh your options and go with what you believe to be the best choice.

I recently got a second opinion on my biopsy and I don't understand it maybe you or someone else can help me. Here's how it reads."It involves approximately a 90% of the sample. The grade 4 is 4 mm. Gleason score 3 + 4 = 7". I don't understand grade four is for mm? I am trying to find the volume of the ...4! It seems to me that they're giving me the measurement! I don't know how big the core was. Please anyone I need help I went to a couple prostate hotlines and nobody already and they couldn't even figure it out.

Don't neglect to counsel patients about the horrific quality of life destroying and life shortening side effects of ADT CASTRATION. Men are never warned about the Severe brain fog, dementia, muscle wasting, frailty, diabetes, osteoporosis, and permanent penile atrophy, etx, etx, etc. I ain't just hir flashes

My urologist is female and smoking hot. It does make the exam a bit more tolerable. A lot more, actually. If she should find an engagement ring up there next time, I will either get married or disavow all knowledge of how that got in there. However I will reclam the ring if the answer is no.

I want a pet scan after my PSA score came back for the first time I’ve ever taken it was 6.2 , and six months later it is still at 6.2 which is probably just my normal score. Also it was sneaked in by my new pcp without my knowledge and without me asking for it. I am asymptomatic. They then scared me off to a urologist who continued to try and scare me and I asked can I get a pet scan and because Baylor Scott White owns the insurance the clinics the doctors and the hospitals they don’t want to pay for it until you are already going through all of the other test which I refuse to do and so I mentioned what if I want to Pay for it myself and that’s when we hit radio silence from everyone. We should also mention the PSA test is almost worthless at predicting metastatic prostate cancer even the man who discovered it Dr. Albin has a very good lecture on this you can find here on RU-vid of why it should not be used at all unless you have already had prostate cancer and a radical prostatectomy. The truth is they don’t want to be cut out of the money that they’re going to make by doing these risky procedures that are worthless if it has already spread instead of using something like the PET scan to begin with. I probably don’t need to do anything but now that they have scared me I’m willing to pay for a pet scan myself I just have to find somewhere to do that and probably just waste some money for nothing. The American healthcare system is a joke.

Thank you for your insight. I had my prostate removed 3 moths ago I'm going for a blood test soon to see if my PSA is zero. I will admit I'm scared my Gleason score was 6 +7 I heard there is a high risk factor of a reacurrance,

Very informative presentation. Dr. Klotz you mentioned the PSMA PET scan in your initial overview but I don't think you ever spoke to it directly in your presentation.

This is my biggest fear headed into radiation. I have zero dysfunction at this point. And this is an important part of my life. And being single makes this that much harder not having one woman to help deal with this.

Side effects from docetaxel were definitely milder than I had feared they might be. They did get progressively worse with each infusion. I had six cycles, and was very glad not to have a seventh. The following year I had six cycles of cabazitaxel. In my case, the side effects started sooner and were stronger. Again, I was very glad not to have a seventh infusion. It took me a couple months to feel like I was back to normal. Some side effects have lingered, but they are tolerable. I got very good results, so it was worthwhile to endure the treatment.

the problem with the idea of shared decision making when it comes to prostate cancer is there is no coordination of care. urologist says have surgery. radiation oncologist if you are lucky to see one tells you about that option. nobody neutral to help the patient make the decision. A coordinator to help w/ the decision should be standard of care.

I am doing the research for my boyfreind who was disgnosed with prostate cancer 2 years ago. He is finally (I hope) ready to deal with it. I don't have a penis myself but I have observed that the relationship between a man and his dick is a very deep one so I can understand his fears of losing his erection at 57. That's what has been holding him up for these two years. This video is wonderful and educational and you can best believe we will be watching it together ASAP! I'll carry him to the doctor on my back if necessary but It's going to happen come hell or high water. Thank you so much for this video. You have no idea how much it helped me. And now it will help my boyfriend whom I love very much.

Dr. McNeil, That was a masterful presentation! I am a Genitourinary Nurse Navigator at a regional cancer center and deal with patients with prostate cancer every day. And I especially appreciate your encouragement to take time to educate yourself and have those conversations. Patients too often feel that once given the "C" diagnosis, it must be removed as quickly as possible.
