Joe Cobb always drops a stealth boy too, so I usually kill em for making grabbing some harder to get (cause of enemies) unique weps early on, much easier, just pop a stealthboy (ratslayer for the rats, Paladin toaster for the night kin master/nightkins etc) and alot of other goodspringers mention you killing him (trudy does, sunny smiles does etc)..

The only problem here is: In Survival mode you need sleep, food, water ( everything a synth never needs but sometimes chosses to have ) but you actually need it so there is no way that he is a synth.
