I wish this type of health service was available in other parts of Ontario. When you look at wait times of 12 hours in ER or 5 hours at a walk in clinic, this seems to be a much better approach.
Suggestion from an old retired RN: Can health outcomes be captured & measured within a more progressive path ( similar to Prochaska's stages of change) to wellness rather than absolute resolution or cure? eg obese patient needs to loose 100 lbs. Agreed first goals for patient is to: 1) address sleep apnea by stating with sleep study 2) attend meeting with dietician to get nutrition education & 3) loose 5 lbs. Results upon follow up 3 weeks later with Dr at clinic: 1)patient does get diagnosed with sleep apnea & using a CPAP & sleeping better 2)patient has met with dietician with plans to follow up however, patient has not lost any weight. Steps 1 & 2 should be counted as a positive outcome. Even a movement from a pre-contemplative to contemplate stage is a good outcome on the path to a person's effective self management of their chronic disease. This results in an increase of the patient's agency and fewer visits to acute care. This would also then motivate a movement to more preventative care rather that reactionary acute care. Today everything seems to be too fast. Commercials run into each other and you never know when the old one ends and the new one begins. When you hear instructions on a phone, they speak way too fast. In this instance, I can understand why older people and others whose first language is not English find it way too fast. I hope that CIHI will take note of this.
This is not right !! elderly need a lawyer ..they have rights to assistance and low income homes first before refugees are getting hotels and our elderly suffer they have a right to live .. thank God my mother had her family speak out for her ... she is still alive today ..thank God

Will they include determinants of health....education, employment, income, housing, social parameters in their model? Will they include health behaviors such as smoking, alcohol consumption etc...otherwise it will miss 70 percent of what keeps someone healthy...not just downstream disease care system utilization....which is what we are trying to avoid.
what would happen if the assessment tools were based on the social model of care versus the medical model of care. I think that there would be more options for housing alternatives to residential care except for those who are beyond the social model of care.After all aging and disability are not diseases so it ought not to be used under the medical model.
