Hi Doc, nothing surprises me anymore, I have pain in my clitoris, and also have to deal with trigeminal neuralgia, A-typical ( which is a hell) it comes almost every night, especially in my dreams, when the brain is most active, I hope one day there is a cure...........

Hi, not sure if I will get a response but watching your video gave me the urge to ask since I am going through the exact same issue for more than 4 months without a single trace of the underlying cause... About 4 months ago, I had a sudden onset while walking underneath my penis area (towards the tip), it felt like a needle spearing through the area which lasted for a good 5 seconds. Pain was very severe and ever since then, I had trouble peeing, walking or just living my daily life normally in general.. ALSO, I also noticed slight discomfort while sitting down on my perineum area where i had to adjust my positioning to find comfort (I didn't take this as serious as penis pain was more prominent at that time). I have since then, done around 20 Urine tests, countless Blood tests, Ultra sound on bladder, Urine flow test, even a Cystoscopy which showed clean view of my urethra and followed by MRI scan on my Prostate and Lumbar Spine. All of these tests showed good results (Only a little protrusion on L4/5 disc). And just recently I have not been able to sit down at all as the pressure on my perineum area has gotten much worse that when I sit down, the pressure then pushes forward and gravitates to my penis, giving a sense of my penis is being protrude and changing its shape slightly, the main problem why i cannot sit down is that the pressure triggers the exact same pain at the bottom tip of my penis where I originally had an onset on. Do I need to go and see Pelvic Floor specialists??

I feel pressure in my pubic area that goes down to vulva.. im always tense there and i urinate alot… i sometimes develop coccyx pain but not anal pain.. i cant sleep because of the pain sometimes.. is it still pudendal nerve entrapment?

@@harryg2453 This requires an evaluation from a urologist. There can be various medical causes for this and treatment options depending on the cause. In the absence of an identified medical condition, pain may stem from an entirely different origin so seeking care from a trained pelvic floor physical therapist once cleared from a urologist would be recommended.
Im curious how severe the pain if have endometriosis... i have dysmenorrhea since when i have my period (i was 12yrs old) the pain for me was too much but when i grew older the pain was not the same as my teenager. Now im 30yrs old and became married the pain almost nothing... im monthly have period and lasts only for 4 to 5 days rarely only 6days....do you think i have endometriosis?

After the removal of my kidney stent... I have been feeling pain inside the hole of my penis. It's been just about 2 weeks.... it comes and goes. I'm concerned.. I hope its not an infection.
It appears that i have overstretched/traumatized my penile nerve from abusing a penis pump, rough sex and masturbation. This has been going on now for over three months and unfortunately i kept aggravating it. It has been three weeks since i have done anything to irritate it, however i am only marginally better. I am scheduled to see a pelvic floor therapist Monday. Does this sound like more of a case of irritation as opposed to compression/entrapment? Are my prospects of a full recovery good? Any estimate on a recovery timeline?

There are many things that could cause penile pain. Your primary care physician should be able to perform testing to rule out a STI. It is possible to have both an STI and pelvic floor muscle dysfunction at the same time, so getting a comprehensive assessment can be important!

That's a great question. It is possible to have the pelvic floor muscles respond to something that changes during menstruation and for that to trigger muscular pain- such as, if you always have diarrhea during your period then you may tend to get irritated pelvic floor muscles due to the bowel changes that occur at that time of the month. But generally, changes in symptoms that occur cyclically around menstruation are often associated with the hormonal changes that occur around menstruation or underlying conditions like endometriosis. It's important to see a provider well versed in this area for a thorough assessment.

Any recommendations of physicians in Dallas DFW who are familiar with this topic and can properly diagnosis me. I am 38 and just started having this soreness in my pelvic region and around my lower back to buttocks area. Ive experienced burning while urinating that comes and goes at times. Thank you for make this video ❤, i had 3 of procedure surgery laparascopy for endometriosis ,hysterscopy and colposcopy . I have done surgery 2 week ago and i go back to my reguraly work in the kitchen (as breakfast cook) but i couldn't handle my lower back pain so painfull also i feel sore in my tummy belly botton after 3 hour standing cooking. We would not recommend using dilators for prolapse. We would recommend seeking an evaluation by a gynecologist and a pelvic floor therapist to give you the right treatment which may include a pessary and specific exercises.

I do have all those symptoms, but I also have pain in my sacrum. Can the sacrum be related to pudendal nerve or to be the cause of it? And my tailbone is tender as well, I cannot sit for too long because it starts hurting.
The pudendal nerve is comprised of the sacral nerve roots S2, S3, and S4. Pudendal neuralgia is often associated with pelvic floor muscle dysfunction which can then be the cause of sacral and tailbone pain. We would recommend seeking an evaluation by a pelvic floor therapist to specifically identify the cause of your pain.
Actually there was a study in the 80s where it actually showed a positive affect in 80%+ of the participants, they didn’t say if it was harmful or not but I’m sure like everything if you do it to much it’ll be harmful

Hi. Can prp work for pudendal neuralgia and/or cauda equina? If so, what location would you recommend the injections? Ive seen several doctors and no one is really helping me. I had a fall and aggravation during PT which led to feet/ankle numbness and urine retention. It progressed to levator ani spasms. Pudendal neuralgia...so a year later and it still hurts to sit (hamstrings & glute pain & tightness and anal/vaginal pain) . I still get deep pelvic floor spasms. Im in daily pain. Im trying to go back to work so i wanna get away from sedative medications (they dont fully help anyway)....any advice please

If you have interstitial cystitis, like the for real no joke IC it is going to impact every aspect of your life and of your families lives as well. You have to be resilient. I'm not saying you can't fall down and cry once in awhile only that self pity isn't a place anyone with IC can tolerate long since this disease causes such a high degree of suffering on its own. There is not one single thing that helps and you need to modify how you do about everything. The best treatment is prevention and that impacts your quality of life so so much. It's wanting more than anything to go with your family and do all the things they beg you to do and having to say no, you go without me and having to pretend that it doesn't hurt you that's your ok with being left behind its also dealing with the emotional toll of guilt ad shame you feel because your letting them down, it's the emotional toll of having your actions not matching up to who you really wish you could be knowing it's not your fault doesn't matter. Your actions speak for themselves. Everyone gets sick of someone who's always sick. The good news is with more awareness most won't go there entire lives not understanding what's wrong they have a name for it and information is there that can help . Don't push your last drops out, is my advice. That makes pelvic floor worse. Try not to reflex anticipatory pain either.
When I apply a very veri very mild sort of force from back to front side aur at that angle which give force from back towards front side of clitoris ...is hurts like just behind its surface there is a sharp pin and it stings painfully.....but update is this that this sharp pain is now occuring at every angle ... This whole thing started may be maximum 2 weeks ago ... please help and tell me what and why is this pain .....I can't afford to go to doctor
