31:24 and 37:34 what about titrating a single dose into two treatments? It seems that you could treat with the initial dose, at say 3/4 the overall treatment dose and once the tumor has started to degrade (> 4 months?), administer a second dose of the remaining 1/4 dosage for a two treatment regimen? One safety mitigation would be the overall dosage does not change, only the treatment regimen and the tumor stage associated dosage. One endpoint could be the scaring residuum. It would be interesting to track the duration of the ‘honeycomb’ tomography to see if by somehow extending this stage the end result is a more efficient and effective tumor degradation. Also, a two treatment regimen may provide a kind of compensation factor for a weaker immune response in some patients. Oh wow. I just reached this part of the presentation! Very similar idea. That’s great.

Hi and thanks for informative presentation. I have gbm that was diagnosed in February 2023. I had surgery followed by radiation and temozolomide for 40 days. I had a good recovery. From July to December 23 i had chemo temozolimide higher doze. But, in decemer 23 i had an mri that a recurrence and doctor prescribed avastin and irinotecan for six sessions each taken every two weeks. Tolerance has been relevatly good. My query is whether is it possible to participate in the these clinical trials after current avastin/ irinotecan?

Can this be used just after a patient starts using Optune or is it limited to patients who just completed radiation? How does someone get accepted to clinical trial?
The standard device can not be used for DIPG as the electrode would have to be placed too low on the neck making it uncomfortable. I have heard that they are developing an new style of array that ight work in that area but it is not available yet. Sorry.

It is not available at MSK. This particular trial is at clinicaltrials.gov/ct2/show/NCT05362409 and a similar trial is at clinicaltrials.gov/ct2/show/NCT05370508 2 different approaches to the same concept. We do not know which one is best yet but both are worth trying
There are a lot of treatments that some doctors use and others do not. This is a relatively new technique only used by a few people. Same thing with Optune. Most doctors use it but others do not like it and won't tell you about it.
Because dcvax-l DEPENDS on the patients own immune system to work and the health of patients vary greatly, the results should be considered with that in mind. A doubling of OS is amazing. Combination therapy should provide a much greater benefit. Eliminating the toxic SOC should also benefit DCVAX, a combination with Optune.. very interesting.

My dad was just diagnosed with stage 4 lung brain metastases. He can't have surgery to remove his largest tumor (3x2in) because its connected to his blood brain barely and if he had tumor removal surgery he would bleed to death. My mom is killing herself getting my dad everywhere he needs to go. They have to travel to Houston on different days, sometimes having to pay for hotels for days and they spend so much time driving from doctor to doctor. They are down to their last $100 because they have been spending so much money on premiums at every appointment. It's been very stressful and hard on both of my parents. It's been hard on all of us. We can't lose my dad. My parents are best friends and they have been married for over 40 years. If we lose him then I would be losing both of my parents because my mom says that she can't go on without him. It would break her for the rest of her life. I'm praying that dad gets accepted into your clinic and I'm praying that his insurance will cover it.
