There are people who experience anxiety in regard to this. Allowing people to be in their own comfort zone instead of forcing people to do something they don't want to is a classier approach.

No, you shouldn't need to introduce your self when recieving an award or about to deliver a lecture. Usually the introductions are preplanned by the MC with the award recipient or guest lecturer.
I don't know if I would call that controversial, perhaps, it is only your impression that no one pays attention to the first part of a presentation or any speech, and if there is no actual data to support what you are saying, it's very hard to make an informed opinion about it.
I think it depends on how useful that information is for the audience (e.g. to confirm you are attending the right presentation at a conference). If it is a business meeting and contact info is already incl in the agenda or a copy of the presentation, then no. It is redundant.
