Previous icu nurse here: I’m gonna guess either versed or etomidate We don’t want to paralyze lungs given use of duo neb and patient is improving. But hence sleepiness, we want to minimize anxiety and WOB by relaxing patient. Thereby yielding safe conditions for intubation & minimizing WOB and or general movement to maximize O2 sats. Sounds like classic asthma, especially given age and lack of context of potential comorbidities.
I don’t even have the balls or stomach to write how horrible getting my tonsils out and the recovery. Let me just say as a guy I haven’t cried due to pain since I broke my foot and that was 12 years ago. The pain and taste was so bad I don’t wish this on my worst enemy. Fuck man it’s no joke.
I was a ICU nurse first, I learned how to be a ER nurse by floating, and I did a lot of agency where I would work both ER and ICU in the same shift. Organized chaos in the ER is fun, but a really good unstable crashing patient with a Swan, multiple vasopressors, a ventric, triple H therapy, septic shock, neuro, codes are all fun to. I dig both the ICU and ER. 😊

By the few and short anamnesis you gave us, it could be a ton of diseases. But if you want to direct us to a cardiac diagnosis, being a female, we have to rule out takotsubo.
