Thank you for the question. The video cast was to sensitize MBBS students on importance of skill. I see a lot of students even in premier institutes like AIIMS and JIPMER just preparing for PG entrance without attempting to learn skills. For your question on skill for a medical oncologist... I would say the following 1. Communication skill of proper explanation about therapy and its benefits 2. Identifying a suitable chemotherapy regimen in resource poor setting 3. Identifying a critical adverse effect of a chemotherapy drug is an important skill today since when using new drugs we may see side effects which were not reported in clinical trials ( around 50 % of serious side effects due to a chemotherapy drug is identified only in post marketing surveillance )
Sir some books mention about these like 2/3 or 1/3 of systole while differentiating Hyperdynamic n Heaving apex...like they say Hyperdynamic is when apex is shifted down and out,and is forceful but ill-sustained ie lasts less than 2/3 of systole and Heaving is when apex is forceful and well sustained and lasts more than 2/3 of systole...is this correct sir?
Yes books give various descriptions.. But we need to understand the functional concepts to interpret the sign. Hyper-dynamic is when the cardiac movement is perceived over a larger than usual area. It can be more than one intercostal space or an area larger than 1.5 inch. Extent of lift may not be an appropriate finding to look for . A widely distributed impulse may not lift the palpating finger that much. Heaving is dependent on the lift . Any lift should be considered abnormal. In normal persons apex if just felt with a barely appreciable lift even in thin built persons. In summary Hyperdynamic - look for area of impulse and Heaving- look for the lift. Clinical signs do not have a very hard and fast rules ( some examiners have !)
