Had this recently happen to me , my question what should one do when it does happen ? I was having a really good morning and truck drive says to me sorry man I’ll be more careful next time and that was it but I felt like it wasn’t a big deal to no one .
I used to work at Walmart same sgit happened back in 2005 gladly no one wasn't inside the trailer at the time ot had one items left i almost went in but luckily i didn't
Operator should be wearing safety glasses and a hard hat. You don't wanna know the amount of times I have had broken bits of timber pallet rain down on me from above during this operation
How is it the drivers fault at that point?!.... the whole place is a catastrophe waiting to happen because it's stacked way to high aslo anything above ground level should be wraped.
I worked at Pepsi Bottling in Burnsville decades ago. As the kid they'd give me some throw away gloves, a shovel and a bin. The driver may or may not have messed up, but we have nothing else to go on. s**t happens. At Pepsi they seemed to shrug this type of accident off. I was young.
I never drive forward unless I am going to drop off a pallet. I always honk to let people know I am gonna enter to drop off. Always need a line of sight.
I happened to me. I backed in the dock after 1 h went and asked if they are done loading me as i was the only truck in the whole yard. They said yes i pulled out fortunately the fork lift was on its way to be in
We are all responsible for our own and each others safety. Both at fault, but as the driver of a massive lump of metal, the driver would be held resposible.
The guy with the load is at fault. You never drive a forklift going forwards with a load that obstructs your view. You drive backwards and you turn your head or use your mirrors.

When I started in the industry engines seen were Continental, Moline, Hercules. Rebuilding or reconditioning engines was something that a lot of revenue for us. I can't remember any 4Y engines being part of that.
In my opinion, he was actually doing good. Seems like he needed to be a little more patient. That equipment is very powerful and can easily get away from you.
