With due respect, what is the purpose of bolus insulin isn't it may cause sudden shift of potassium inside cell and decrease in glucose osmolarity might precipitation of cerebral edema why not to directly start with FRIII, kindly answer this ? thank you
Thank you so much for your comment. The Total insulin requirement is broken into bolus and infusion. Going only for infusion will lead to very slow decrease in blood sugar leading to adverse outcomes. The above protocol ensures correction of blood sugar without causing massive shifts in K+ . The massive shift in K+ levels happens when the entire insulin requirement in the patient is given as a bolus dose.
