Sir, I have a question. Doctor is confused, that's why I ask you: At the moment, man 80 years old has huge diffuse abdominal pain in every few hours hitting its peak, also having nausea few hours after any food except rice, that started 15 days ago as elevated temperature up to 38C, and elevated Ly, and lower Neutrophils. First 5 days there were just high fever and nausea, no pain. He was laying all day during that period, no movement, no stool, urination normal. At 5th day, big pain appeared, and he went to emergency. ECG normal. Lung sounds normal. Ultrasound showed many gass, and gallblader was filled with "muddy fluid". Also elevated gamaGT at that moment. They sent him to hospital for furter diagnosis. CT showed no non-organic calculus in gallbladder, no distension of gallbladder tracts, and potential ulcer in abdominal aorta. (Maybe holesterol, organic, calculus?) No extravasation of blood from aorta. But he feels pain relief when gas is released. CT showed no gastrointestinal obstructions. Only gass on ultrasoumd mentioned. Now, he is on pain medications, and spasmolytics, and glycerine rectioles. All help him but not enough to relieve pain. What do you think, sir?
@@nadezdadjuricic1558 the post Randal pain with acute exacerbations is Biliary colic ..the USG shows ,muddy GB ( this is biliary sludge ) Ct is having no value to detect Gall bladder problems . Either MRCP or HIDA scan ..should be useful here .. Presence of SIRS ..with such a symptom complex confirms acute cholecystitis ( according to Tokyo Guidelines)

@@afterdark2023shorts private medical colleges usually are not focused on academics (except DYP /Manipal and few others ) .Rest ..don’t have good infrastructure and faculties.. Yes ..u can opt for private ..but u need to shortlist the colleges ..visit them ..talk to the seniors there and personally create an image ..
Sir i have a rank of 14k 🥲 , i don't have much choice but would probably get MS General Surgery in my own state only , but the PGs there say that they don't get much hands on 🥲. But I don't want to drop since im getting MS General Surgery in a Govt College and the Increasing uncertainties in NEET PG but at the same time there's a FOMO of not being a good Surgeon after my MS 🥲. So how drastically would it matter ?

My daughter had fever in November 2022 after that showing swelling and pus filled in neck, later it was removed and all reports were came negative. Then the same incident happened in February 2023 and again in the same way Dr Puss was removed but again all the reports were negative..again same incident happened in August 2023. And now the same problem is came again to my daughter...we have MRI CT scan blood cultures and pus cultures but all reports showing negative doctor also don't know why it is coming and coming...so please help me in this case we are too much suffering...pls help us..
Excellent insight. Myself a Mch surgical Oncologist I think Mch surgical Oncology is still having that general surgery overview and in more refined way as we do head ti toe. They can open a nursing home and and can manage too as surgical Oncologist don't have a tunnel vision like any other superspecialization rather a broader view
