I played two times without dying and the trophy did not pop. That would be my last missing trophy. Hmm. What did I do wrong? Do you have to play it without saving in one go?

fucking brilliant!! thank you so much for this. my buddy and i followed along and got all achievements in just under 2 hours (i messed up a bit in joining and selecting the proper character)... [pro tip: if you accidentally start as a female - exit. go to main menu, craft and save a male character, go back into game - you will notice you are still female - have someone kill you. you will respawn as the new character you created.] thanks so much!
