I’m a nurse and it looks ingrown and or infected to say the least to me. They may just have to pull his toenail off and treat with antibiotics and cream/ ointment wrapped to protect it till it dries up. When his toenail starts growing back up buy a bunch of bandaids that are flexible so the toenail will not hung on his socks. They may give him anti fungal medication to take too just to make sure they cover all bases. Hope this helps. ( plus I didn’t like the lack of color under his nail bed) Get it checked out asap.
