30" is the height requirement in California. The federal height for most other places is 48". Any work done above that height will require some sort of fall protection. Guardrail is an excellent choice, since it doesn't need to have regular inspections, testing, or recertification, and users don't need special training in order to be protected.
So I've just been assigned the job of monthly Ladder inspection, & everything is fine, but there is this one ladder I have questions. All other fixed ladders have 3 points of mounted, but that one is only mounted on the top & bottom with the mounting bracket unattached. It's very jiggly & if it fails the user would fall off a roof.
That definitely would be cause for concern! The ladder would need to be updated so that it's secure and safe. If the ladder is updated AND is over 24 ft., it will also need a PFAS (personal fall arrest system) installed to meet OSHA guidelines. Any ladders installed after 2018 or UPDATED after 2018 need to meet the new ladder requirements.
