With the exception of the great discovery of an LMHR, I find arguments between the two camps silly. Some people thrive on one diet, others on the opposite. Each person needs to find what keeps them the healthiest. This is done through trial and error and exposure to all sides of the argument. I personally appreciate both sides and everything in between. You are all brilliant people and we continue to learn a great deal from you through this medium which did not even exist just a few decades ago. For me, the search is not to find a diet that existed eons ago and replicate it. It is to find a diet that works TODAY (with all the food alterations we have now) to extend our lives beyond that which nature allowed in antiquity. We ALL want to do better. To extend our lives beyond what we thought possible, don't we? If not, why not just go back to being scavengers? That came before hunters...why choose the paleo era? Obviously, because it was better. Well, maybe today we can do even better than that with the help of science and rigorous study. Thank you all. Your tireless efforts are not unappreciated.

Absolutely effing STELLAR!! 💥💪 Will be contacting Dr. Diamond privately in the next day or two to discuss the influence of thyroid hormones - specifically T3 - on serum lipid levels. We are long overdue to have a nuanced discussion about this in the keto community. Looking forward to your conference next year, Dave. I didn't come to this first one, but I won't miss the second one, my friend.

Thank you Dr Ali on your answer re drugs like Repatha. Getting off statin and MD is talking Repatha, but LDL 70 or below doesn't sound healthy to me.

LDL on its own means nothing It’s inflammation that caused heart attacks My LDL is very high after 18m keto but my HDL is 3x that and I have zero inflammation

He should stick to his field. Raised cholesterol is the greatest predictor of heart disease,full stop. Get it down at all costs. Take statins like I did if you can't control it by diet and save your life. My heart attack and bypass was 26 years ago when I was 57 and cholesterol through the roof!

weel it sounds like the discussion is calling keto fairly new, but wait, 125 years ago there were no processed foods, no white sugars like we have, no gmo foods or wheats and rices and manmade carbs that are processed, humans ate animal, cheese, plants and Cancers and diabetes were much much lower, than now in our so called SCIENTIFIC age... the type of hysteria about cholesterol didn't cause this panic and medications that are hurting other organs such as statins, the sciences are all lopsided due to the medical powers that be. We have all learned this. The drug manufacturer's are in control...and we are all dying as a result of the greed. Face it and eat fresh non processed veggies, meats etc and cut the carbs, they all turn to sugar

I think I fall into the Lean Mass Hyper-responder type. I have been carnivore for five years and keto five years before that. I have triglycerides of 0.6mmol/l (53mg/dl) HDL 2.4mmol/l (93mg/dl) and LDL of 4.5mmol/l (174mg/dl). I have had a CAC scan and it was zero. I am nearly 62 years old. I have a strong FH of CVD, dad first MI at 48yo. Do you know of any research teams in Australia (my location) who are looking for people to follow. The more research and information we have on low carbohydrate eating the better to re-educate the masses and stop the fearmongering.

This is just nonsense no element of cholesterol linked to death has ever been true except in tiny sub 100,000 participant studies, look up: "Higher LDL-Cholesterol is Associated with Greater Longevity" "inverse association between low-density lipoprotein cholesterol and mortality" "Total cholesterol and all-cause mortality by sex and age: a study among 12.8 million adults"
@Gumballa87

So... the problem seems to be, there isn't enough data on how a healthy person functions on a diet that has been the norm for all human species (plural) for millions of years minus the last let's say 20.000 years with the onset of year round carbohydrate availability. Of course nobody pays money to figure out how "healthy" works.. maybe insurance companies might have an interest in that?

I love love that you and Nick are really shedding new light on Cholesterol and lipoproteins. Dave, you are correct to pause and reflect on statistical methods used to model something.. this can be so problematic across so many areas of science. Keep pausing and reflecting. An observation I would like to make is that using isolated particles or molecules as measures/biomarkers/causes of mortality all cause or heart disease is super problematic in the absence of systemic view of the roles these particles play. C27H46O is a molecule of mammalian life that has so so so many roles in our bodies that it is extremely difficult to tease things apart. You know all these roles very well, I'm sure as they stretch from repair, to building blocks for cell membranes, to precursors for much of our hormones, to the essential building block of Vit D. If we then look at Lipoproteins it is even more amazing since they themselves have complex functions with in our bodies as carriers essential to enabling everything. ApoB is a marker on the lipoprotein that allows it to be recognized by cells in the body that want something from the lipoprotein carrier. What is missing in this type of analysis is an appreciation of how lipoproteins rise and fall for so many reasons it is hard to even keep track of them. This is like trying to hit a moving target in a system so complex that it rivals our own galaxy. I wish you guys would take a step back and follow the path of Dr. Nadir Ali (Cardiologist) who spends so much time teasing out the actual functions and behavior of these essential molecules of life, rather than constant analysis of associative or correlative relationships with Disease X. Take a step back look at the bigger picture and that will provide the important answers we are all seeking. THe body makes all these molecules for a reason. There is no way that they are causal. Insulin does damage to the human body when in excess, but ultimately it is not causal. The root cause is something else. Nick takes about ApoB in a causal chain.. and already I know that he has lost sight of human evolution and is now following the path of allopathic medicine which cannot ever see the big picture.

@mowthpeece1

There will be a GENE that protects the heart. I have that per my DNA tests. After he wins the Nobel, they will isolate that gene and create new medicines that actually work. Mark my words. I hope Mr. Feldman becomes a billionaire! But I was shown years ago, by 23andme, that I'm one of those folks with the genes. It's part of the longevity gene collection. My mother died at 97 from covid, her sister is 102. My father died at 91 from colon cancer after a lifetime of alcohol addiction. His father died at 99. No heart disease in either one. This is GENETIC. Test those participants for longevity genes!

I know it is always recommended that “ we check with our medical doctor” but that is usually why we are all here. I am a nurse and my medical doctor and many medical doctors are not up to date cholesterol and nutrition. A visit to your medical doctor for any issue usually results in a prescription for a medication. Thank you for the information and research you all provide!

So if arteriosclerosis is not axiomatically driven by high blood lipids, what then causes it. High organic acids ripping through the vasculature? High systemic inflammation? If it is caused by high systemic inflammation what are the top three most common drivers of that systemic inflammation? (Leaky gut? Immune dysfunction? High load of chronic infections like HSV, EBV, CMV?) Please research this aspect of the problem and let us know. Thank you for all your hard work thus far.

I recently was told that I needed to go onto statins as my ldl is too high (4.4). He didn’t bother to asked me about my diet. I told him I don’t eat junk food, not fatty food, lots of vegetables and has maintained a very healthy diet within the last two years. And on top of that I exercise between 4-6 days a week and I have never felt better in my life. He simply told me that unfortunately it’s the gene? I am 47yr and compared to my friends i feel like I am in my thirties. He never bothered to explain to me that my triglycerides is low (1.0) and my HDL is high (1.2), which puts me in the “isolated LDL” category. Thank you Dr. David, otherwise I would have taken the statins.

The issue that isn't addressed that I'm dying (no pun intended) to know is: what about those folks who already have plaque and a high, or moderate, CAC score but have been following the low carb guidelines for a number of years, resulting in low trig's., high HDL & LDL as well as large LDL particle size? Should THEY be concerned? Or might statins be a consideration for THEM?

Hopefully the citizens learn about real science and not assumptions. Dave, please stop assuming ketosis and "keto" without measuring. Assuming keto/ketones/ketosis is not science. You used the word "keto" 66 times in your paper without measuring a single ketone. Is that good science?
