Begin with adding 1 tsp salt to the subji first, followed by tasting and adding some more if required. Also, the Fenugreek leaves (methi) will shrink to 1/4th or 1/5th in volume while cooking. So salt has to added considering that.

No water is required. Either use a thick bottom pressure cooker or cook on medium slow heat.

Only ghee is enough. Ensure it is cooked in a heavy bottom cooker or on a medium flame.

Can I use flour instead of semolina?

Rawa/semolina is coarse and grainy, so the appam will be soft. If any flour is used, it might become sticky and hard, so will not recommend it.
