Rarotonga has daily flights to new Zealand I think and there used to be a Los Angeles Rarotonga flight once weekly years ago(I personally took it in 2 occasions in 2007 and 2010) but I'm not sure it still exists. To get to Aitutaki from Rarotonga there are several flights a day but to the other islands they are more rare and to puka Puka extremely rare: maybe 4 times a year?not sure. Most likely you will have to charter the airline plane and share the costs with other passengers
