Please talk about spinal cord injuries that are caused by compression over time like myelomalacia & how it causes permanent myelopathy. There isn't enough discussion about this even after ACDF is preformed.

Getting one hand done next week. They make it sound like I'm going into surgery where a whole crew will be assisting. Told me I have to have somebody drive me home. Hope not. My doctor said it is a 5 minute operation. Pre operation visit on Monday. Thanks to this video I will have a lot of questions for them.

I was watching this to learn about the procedure as I am having trigger finger surgery in two weeks. I had to stop watching because your music was so loud I couldn’t hear you speak. I wasn’t a good song either!😂 Perhaps no music next time, it seems to cheapen the procedure. I’ll have to watch another RU-vid video where I can hear. Otherwise, I did like your explaining everything as you went along.

Dr. Blum is the best. I had my index and ring finger operated on for trigger finger three weeks ago. What an amazing experience. I now have full feeling in my fingers and no trigger finger. I found Dr. Blum to be kind, compassionate and a skilled surgeon. The office staff is excellent as well as his surgical assistant, Jennifer. Thank you Dr. Blum for your expertise and kindness. My Aged Mother in her early 80's at the time, went for a "Revision Knee" replacement. But, it became infected, and she had to go for what they called a "Washout" but this is when her "Real" problems started. She was driving at the time, but here in the UK where most cars are "Stick Shift" she began to select the wrong gears, either revving the engine, or causing it to stutter. I don't drive due to medical reasons, and said, "Mum what has happened" she had been driving since 1975. Then she began to either oversteer, or understeer. I told her she would have to give up driving. But she began to deteriorate, to the point that she lost both arms, and eventually her legs. She became incontinent at both ends, even losing her speech at the end. She was placed in a "Nursing Home" where she eventually "Passed Away" We went to her Funeral only last Friday April 19. If anyone out there is told they have "Cervical Myelopathy" and Surgery is the only option. Then get it done, my Mother, before she lost her speech, wished she had opted for Surgery. She may have still been here now, if she had the Surgery, to reverse the Spinal Cord compression in her neck.

@@joerobert-qe9cn I have a second lot a scan's, neck motion X-ray, CT, and MRI done this week and the surgeon is going to let me know what options are available. Surgeons have so far suggested surgery to prevent further deterioration. However, they said it will not fix the current problems (neck, shoulder, arm pain, dizziness, instability (I close my eyes and it is like I am drunk). This all started last year (October) while I was at work. I went to lift my arms and pain ran down my neck, shoulders and arms, it was so intense I could not move my arms for 15 mins. Lifting anything set the whole thing off again. Not looking forward to any surgery (scary) Cheers.

I did my research and found out that taking vitamin coenzyme vitamin B6, P-5-P (Pyridoxal-5-Phosphate) 50mg - 3 times a day along with Chelated Magnesium (as magnesium glycinate) for absorption, cured my trigger thumb! From my research they said you have to take this for the minimum of 6 months to start working. Well, 6 month and 2 weeks later my trigger thumb totally cleared up, seems like overnight after suffering for over a year and a half. I'm cured, and just last month my hand doctor said he thought I should have the surgery to fix my trigger thumb! My trigger thumb was really bad, had to splint my thumb 24 - 7. I even had steroid shots in the thumb, no help at all! Anyways, this worked for me, even though my doctor didn't believe that it would work! A major resource for my research were 2 books, one written by John M. Ellis M.D. & James Presley, titled, "Vitamin B6 The Doctor's Report" copyright 1973 and "The Doctor Who Looked at Hands - Vitamin B6" by John M. Ellis M.D. - copyright 1970.

What about leaning to one side? :')
@ocsfdocs
@ocsfdocs 11 месяцев назад
Leaning to the side can certainly help to decompress the area of the spine where the nerves exit the spinal canal to innervate the skin and muscles of the leg. When an individual suffers from a strain/sprain or has nerve irritation it’s not uncommon to lean in a direction to help take pressure off the sensitive nerves/muscles. This can help alleviate the pain/muscle spasm that may be occurring.

My back feels better when I do the opposite, lean it backwards
@ocsfdocs
@ocsfdocs 11 месяцев назад
Great point Andrew! Depending on what structure or structures may be irritated/injured flexing or extending the spine may feel better. It would require an evaluation by your doctor to better determine why that may be the case. Spinal stenosis is an example of a conditions that frequently responds positively to leaning forward and flexing the lumbar spine to take pressure off of nerves.

From the information I have seen, the triggering is caused by some obstruction on the tendon. If the ultra sound gives you the visual, why are you cutting the sleeve/pulley instead of trimming away the obstruction on the tendon? Thanks.

Just he’d my procedure today. About 5 hrs ago, and I’m feeling it pain wise. I can’t make a fist . The palm of my hand is so sore and the A1 pully area. During my procedure I was first injected with a lot of numbing fluid, then my Dr was poking around with a needle which felt like forever. to make sure I was numb. Once the device was inserted, I felt pressure- and the were 2 majorly loud pops!! Like everyone looked at each other!! Dr said that was the sheath it was so thick. Im having anxiety thinking the tendon was mistakenly cut. I never had the catching from my trigger finger. I think mine developed from a broken PIP joint I had. My whole Hand was messed up from that. I had pain stiffness , popping and cracking in my palms, finger was starting to stiffen up every morning. Hopefully this works, - otherwise I don’t know what to do!!

@@oshobaadu6272 I’m from NYC, for me, in y case it was a mistake. Reason being I had three small nodules in the palm of my hand, the were tight but not bothersome. Unfortunately they were Dupuytrens nodules. After the surgery, they got worse, increased in size and pain, and started pulling slightly. I eventually went to a hand surgeon and had them removed, (Dupuytrens excision, and a flexor tenosynovectomy, for the remaining inflammation from trigger finger. My hand will never be the same, but currently I have it to a point that’s manageable. As I stated for my case, looking back, I would not have done it. If you have nothing wrong with your hand other than trigger finger, you should be ok, But there are risks from this procedure, technically Dr is going in blind. Yes they use the ultrasound guided, however, after my experience, I would go open, even with longer recovery and I don’t care so much about scarring as long as I feel relief. Open they can see directly everything going on, they can rely high enough but not high to cause any bowstringing. That’s just me, the success rate is very high for percutaneous and there’s data proven it’s effectiveness. just know the risks. Make a well informed choice. Best of luck..

Excellent technique. I have had two trigger finger releases several years apart (ring fingers) under local anesthetic. One went very well and had another hand specialist perform the other which went wrong. Several weeks after surgery something snapped at the surgical site, felt like a rubber band snapping on my finger. Three months later would still have a small divot that wasn't healing and what looks like a beige rubber band partly sticking out of the wound. The hand surgeon probed the divot with a hypodermic needle and said everything looked fine. Three days later I had to have emergency surgery because the wound was severely infected (salmonella). I had to undergo physical therapy due to the loss of mobility of the finger, which years later still can close the hand completely, and had a lot of scar tissue at the site.
