Queen Mary and Queen Elizabeth, the Queen Mother, always curtseyed when introduced to a reigning Sovereign. Even Princess Elizabeth, when representing her father, King George VI greeting the King and Queen of Denmark curtseyed to the King of Denmark. On formal occasions, the Queen Mother would curtsey to her daughter Queen Elizabeth. While the Pope is not on an official State visit, he is, just the same, a reigning Sovereign, so the Queen of the Belgians would be expected to curtsey to him on being introduced.
