Thanks for your video. I recently had an extraction of a molar and bone graft for a possible implant in the future. The oral surgeon took some blood to mix with whatever they were using for the bone graft. I did not know that there were several types. I am on osteoporosis medication. Would that be the reason why they used my blood?

Thank you for such a thorough video. I didn’t realize how dangerous upper molars. Also, you saying that you need to keep an eye on the root canal (regular appointments with x-rays) for the rest of your life was very good information.

Hi, great work. I’ve been thinking of getting snap on dentures, on both up and down sides. About how much can a procedure like that be? I know prices can vary, but what could be an estimate having also to remove all teeth. Thanks 🙏🏼

Hi there. You want to keep your teeth for as long as possible. If you can even hold on to even 2 to 3 teeth per arch, you don't need snap on dentures. Regarding the costs, it can get pretty expensive in the US. You have to pay for the tooth removal, implant placement, and the snap on dentures themselves. You're probably looking at a few thousand dollars, possibly more than ten thousand, for this type of treatment. Obviously, prices vary significantly based on where you go. Good luck!

hello sir .pls pls advice me.i had extraction of 2nd last lower jaw molar tooth 2 months back.my dentist is suggesting to go for implants.for which i am not ready.becoz i do not want to do drilling in my mouth which might have adverse effects on my health at longer stage what i feel.so can u suggest me a partial denture for single molar (2nd last molar)and which denture shall i go for .do not wnt to damage my all other healthier teeth.pls advice me .pls pls .

Hi there. I'm sorry about your dilemma. Honestly, I wouldn't write off dental implants because of concerns regarding drilling into the bone. Remove a centimeter of jawbone won't impact your life adversely. Having said that, if you want the most "holistic" option, then don't do anything. No implants, and no partial denture. Dentures do more harm than implants as they damage teeth, etc. They are your 2nd molars, all the way to the back? Leave it alone. No dentures, no dental implants. Good luck!

Dr. Are there non-epinephrine shots i can take for a molar 19 root canal that can be effective? Here i had a 1st shot and i dont know if it was anxiety but i soon started to feel heart rate go fast and was scary.

Hi there. Great question! Sure, there are no epinephrine shots available. The problem is, these shots also wear off really quickly. Like in 20 to 30 minutes. Which means they only work for fillings, and not for complex treatments like root canals and tooth extractions. Plus, if the numbness was to wear off that quickly, you'd feel a ton of pain afterward. You have a few options to reduce your anxiety: 1. Ask your doctor to administer the shots slowly, over the course of 20 to 30 minutes, while monitoring your BP 2. Come up with an anxiety solution, like taking an anxiety pill or nitrous gas I've used both options with great success. But you need the stronger shots for the root canal so you can't skip the epinephrine. Good luck with your root canal!

Having three failed root canals and getting them redone been a roller coaster if health issues, antibiotics, bacteria becoming more resistant. A torture and thousands of dollars on supplements to staying healthy. Never discussing allergic reactions, bacteria growing on the areas becomin more resistant, my body rejection of this dead tooth from day one root canals done.

I went to doc last wk... I had a rt canal done 2x on the same tooth. In later months and years... ive had minor to severe pain due to infections. Doc just gave me antibiotics...if it doesn't work, she could pull it as a last resort. I have a root canal infection right now!

Hi Kristinne. Sorry to be blunt, but you should remove this tooth ASAP. Redoing a root canal is already a terrible option. However, if you redo the root canal and the infection remains, the tooth will become permanently infected. The antibiotics only mask the symptoms temporarily, as your tooth infection will never go away. Get this tooth removed and rid yourself of the infection and then worry about replacing the tooth.

Hi Doctor, you mentioned below that taking periodic CT scans is not a safe or effective solution either. When you said CT scan, I assume you mean CBCT (Cone beam computed tomography) ? Why do you think taking periodic CBCT is not safe ? Are you worried about the radiation ? Thanks.

Hi there. You are correct, I meant CBCT. The truth is, no one actually takes annual CBCT to monitor their current root canal conditions. Too much headache, too expensive, too much radiation, etc. You might be able to do it for the first few years, but chances are you will stop doing it after a while. Then, should an infection brew up 5 or 10 years down the line, it will continue to go unnoticed for a long time before it is diagnosed. If it ever is. So yeah, this may be a way to monitor root canals in an ideal world, but it doesn't work in real life. Just my 2 cents. Best of luck!

Hi I just went July 2nd he extracted an eye canine tooth in front under bridge. 2 to 3 days later I got dry socket. He packed it then 4 days later took it out less than week later developed dry socket again 😢. Now after packing it again, about a week later he took the packing out. Now they are talking bone graft under my 7tooth bridge. I heard it's not necessary, because I'm not getting implants. What do you think? Do I really need bone grafting? It's alot more money. Is he taking me for a ride?? Thanks

Hi Pat. Sorry about all of your headaches! Your case sounds quite complex, so I can't pass judgment on it. However, I think getting a bone graft is not a bad option. I know you're not planning on getting dental implants, no one ever does, but your bridge could fail. Most do. At that point, you may have no other option but to receive a few dental implants. Having the bone graft treatment will be beneficial should this happen. But I would need a lot more info to tell you about what's going on with your situation. Good luck with your treatment!

What would be the process to put the four post in the snap on? Is that possible? And does that extend the life of the teeth themselves?

Hi Leticia. The implants are placed during a routine surgery. Your dentist will drill your jaw, insert the implants, and allow them to heal. Then, after a few weeks/months, they will place a connector and attach the implants to your snap on denture. I'm not sure what you mean by "does it extend the life of the teeth"? The denture, whether it's snap on or regular denture, can last for many years. Some people never get a new denture. Others replace their denture every 5 to 10 years. Most people who receive snap on dentures, use the same denture for as long as possible. Snap on dentures are more difficult to make, and they are quite expensive, so most patients who get them, use it for the rest of their life. Don't lose your dentures! Good luck.

Some people are afraid of the dentist because their parents took them to the cheapest dentist instead of the best possible dentist. Remember that if you are now an adult living independently, you have the power to pick your own dentist. Ask as many questions as you feel necessary to allay your fears when looking for a new dentist.

Hi Willis. I agree! If you are okay settling for any dentist that accepts your insurance then you might have limited options. But if you are paying cash, or have good dental insurance, then you should test your dentist's knowledge. Asking a few questions can tell you how knowledgeable the dentist is, but more importantly, if they are honest or not. Hope you found yourself a good one!

When I had my follow up visit three months after having a dental implant installed, the dentist said "WoW!! I have never seen Less union between the jaw bone and the implant post." That is how I found out that I had diabetes in 2017. He decided to take the implant post out of the lower right section of my jaw.

Hi Willis. Interesting! Extreme bone loss can be due to poor implant placement. It could also be due to certain medical issues, particularly uncontrolled diabetes, smoking, and a few other serious medical problems. Good on your dentist to help you figure it out!

Hi Doc, i bumped into your helpful video and just want to know which do you recommend for my 3 missing upper front teeth.. the denturist told me he prefers metal but I also want to consider flexible because they said metal can damage your existing teeth.. your advice would help me to choose which one. Thank you!

Hi there. Personally, I prefer flexible dentures for replacing a few missing front teeth. The flexible denture looks better, and is less likely to harm your remaining anchor teeth. The main disadvantage of a flexible denture is that it's hard to add teeth to the denture. This means you should not be planning on losing any of your front teeth for the foreseeable future. I have a 3 unit dental bridge and it’s been a year now but i am very worried about the bone loss I don’t have any other issue with my bridge but the thought of bone loss worries me too much , should I switch to an implant? I am 24 right now , how much bone loss can happen in a year or two ? Will i still be able to get an implant?

Hi Van. Stop worrying! If you already committed to a bridge, then that's what you have. Focus of cleaning your bridge, get regular checkups, and make sure that there is not too much forces being exerted on the bridge when chewing (have your dentist adjust it if necessary). The bone loss that happens from a single missing tooth is minimal. You will be able to place a dental implant in the gap 10 or 20 years from now. Here are a few possible scenarios for you and your bridge: 1. You take great care of your bridge and it lasts forever 2. Your bridge breaks after a few years/decades, but your teeth are in good condition, so you replace it with a new bridge 3. One of the teeth supporting the bridge fails Scenario three is the worst one, obviously. At this point, you will need two implants, one to replace the original missing tooth and one to replace the recently failed tooth. This is a simple implant treatment, one that I have performed many times with 100% success rate. There is almost always enough jawbone to place these two implants. Of course, the treatment will be expensive. But there is always enough bone to perform the treatment. Good luck and stop worrying so much!

Hi Eddie. Here is the reason: Upper back teeth are close to the sinus. If the root canal doesn't work, the pus will drain directly into the sinus, and from there to the brain. This will usually not show up on standard X-rays. Plus, because the root canal treatment masks the pain symptoms, you will not be able to tell if there's infection in the tooth or not. So you may receive a root canal that resolves the pain, but not the infection. This results in a tooth that continuously drains pus into the sinuses but with no pain symptoms to inform your body that something is wrong.

All root canals remain infected. There are micro canals that can never be reached and cleaned. They are dangerous! I know because I got very sick after 2 infected root canals.

I didn't think your video was long or boring. LOL! I thought it was very informative! I needed to hear the reality of root canals because out of 11 successful ones, the most recent is I think might be a "Fail" due to the sane continuing pain after the procedure. I went back to the dentist and took more xrays again. The xrays didn't detect any problem, but the tooth next to it showed decay and could be the culprit for pain. I ended up having a RC on that one last week, and it seems fine, but the tooth I was having the problem with still aches and very sensitive when tapping. So now I don't know what to do since no dentist can spot the culprit. So that is where I am now.

Hi Sally. Interesting scenario. Usually when a root canal tooth hurts when tapped, it means that the root canal was successful, however, the tooth itself is too weak and fragile to support your bite forces. This pain is not from the tooth nerve, but from the tissue surrounding the tooth itself (we refer to it as a PDL). If you were my patient I would tell you to remove the tooth, but I'm sure your dentist will offer a different solution. Best of luck to you!

@@johnjazayeri Thank you so much! I appreciate your reply! I've had 11 root canals and this is the first with a noticeable problem. Last week the hygienist suggested I have a CBCT scan when I consult with the endodontist again. As you mentioned, what's causing the sensation may be in the gum tissue. Actually the pain has now subsided, but there's still that "sensation" when it is tapped. (Gosh, I;'m at a loss finding the correct word to describe it.) I was hoping to get it capped because it's not a tooth I want to remove. That would make 3 in a row missing, probably too big of a gap for a bridge, which leaves an implant, I don't know. Or maybe a temporary crown placed on it before the real crown is put in. Well I will see the dentists in a few weeks. See what happens.

In 1987, I have #30 and 32 extracted and a bridge on 29-30-31. In 2001, infection on 31. Root canal was done on 31 and a new set of bridge. Now, infection on 31. Should I do implants on 31 and 30? Or should I do implant on 31 and new set of bridge on 29-30-31. There is noticeable bone loss on 30 since it is gone for 37 years. Please advice. Thanks

Hi Leo. The best treatment is to place a dental implant implant for #30 and #31, and put a crown on #29 (or cut the bridge and keep the existing crown on #29 if that's possible). The fact that #30 has been missing for 37 years is not that important. If there are teeth on both sides of a missing tooth, as is your case, there should be enough bone for a dental implant. Lastly, I don't understand how you can do an implant on #31 and a bridge from 29-31. You can NOT connect a natural tooth to a dental implant. If you were to lose #29 as well, then you could place an implant in positions 29 and 31 and place an implant bridge on 29 and 31. However, if you have a dental implant #31 and a natural tooth #29, you can't connect these two. I hope this helps, and best of luck. Let me know what you end up doing.

Hi there. It's probably because you;re dentist is relying on a 2-D X-ray. You would need a CT scan to identify infected root canals. The infection could be behind, or in front, of the tooth. This will not show up on standard X-rays, and will only be visible on a CBCT. Also, your dentist can probe around the tooth to look for bone loss. It is very common for infected root canals to not show up on standard X-rays. Most dentists just write it off. It's your body, be your own advocate and get a second opinion. I would bet that the tooth is infected based on what you are telling me. Good luck!

Hi Tom. I've seen some amazing results with these implants. I've never worked with them personally. However, I love the idea of using a small implants to avoid difficult, lengthy, and expensive bone surgeries and restorations. Go for it!

Hi there. The answer can vary greatly. Some patients lose so much bone that there's no way to place any implants. Luckily, this only applies to a very small percentage of people. Most patients will still have enough jawbone to place a few dental implants for snap on dentures. If you want permanent teeth, you might have to perform more complex and expensive bone grafting surgeries. Have your dentist take a CT scan and they can tell you how much bone you have, and what implant options are available to you.

Another point to make in your teen years, make sure you get your wisdom teeth out if they’re impacted. I avoided getting mine out because I was scared of the surgery, combine that with bad oral hygiene and I lost almost all my teeth at 23 😮

Interesting. This can happen, but it is rare. However, if you have serious gum disease, then yes, it becomes very important to remove your wisdom teeth. Crooked teeth, combined with gum disease, and pressure from wisdom teeth could cause a similar scenario. Did you also have gum disease and crooked teeth? Thanks for sharing.

@@johnjazayeri my teeth were very slightly crooked. Enough so that I had to get braces at 13. And I was told by the dentist at my consultation for dentures that I had some pretty bad gum disease as well

Nesbit dentures do not contain metals, they are plastic/nylon based.

One tooth posterior dentures are not too comfortable. Their only purpose is to keep your remaining teeth from shifting. You can chew on it, but you won't find yourself using it that often if the denture is only replacing a single posterior tooth.

I plan to possibly have my front four teeth removed for a partial (can’t afford implants or caps which led to this scenario), would the metal partial be very visible since the clasps would most likely be around my canine teeth?

Hey there. You can have the denture extend to the back teeth and place the clasps on the back teeth only. This would require some planning and discussion with your dentist. You can also opt for a flexible denture with pink clasps in the front. These are the most common ways of getting around a metal clasp showing on your front teeth!

Well that means a dead tooth in your gum that is dead. How are you saving a tooth if you get a root canal. It puts dentists who do root canal all day long. He said why a root canal is needed to save the gum abd teeth next to it, well so does an implant.

That is an excellent option. 2 upper implants to stabilize the upper denture, and 3 lower implants so that the tongue can't move the denture around. Probably the best bang for your buck. So long as you are okay with the roof of your mouth being covered with the upper denture.

I had a broken molar and root canal infection, which caused a histamine overgrowth and triggered arrythmias soon after eating. Upon tooth extraction and root canal removal, my arrythmias stopped.

Hello Doc. Jazayeri, I've been watching your RU-vid channel video for Root Canal treatment doc. Hoping I can get an advice to you about my situation of my failure root canal treatment doc. It's been almost 3months now since I have my first Root Canal treatment for my front tooth. April 11 2024 I visit a dentist to have a check up cause one of my front tooth chipped out then after if my dentist advice me to undergo Root Canal treatment to save my tooth cause its infected due to my long time pasta ( filling) she did x-ray and then i did undergo antibiotics for like a week . Then after a few days all I felt the whole week if sensitivity, twinge on my Root Canal tooth, tingling and very uncomfortable teeth bite. Every week of the whole month is just suffering I've been going back and forth every week because I still suffering the same concern problem till now . Last month May23,2024 they did a retreatment for my root canal cause the infection is still there, I feel that this time my retreatment she really dig deeper than the first time that even I am with 2kinds of anesthesia I still can feel the pain, pressure of drilling and digging that she did for the retreatment root canal procedure then after it she prescribe me a different kind of antibiotic which is clindamycin for 7 days than after retreatment.the whole night and the next couple of days I was is very pain , very twinge , sensitivity, very swollen upper lips and the half of my face and I feel numb . I'm very scared right now they give me a phobia for Root Canal until now I'm not yet done of my Root Canal treatment.. Hi there. Thanks for the detailed explanation. I wish you would mention which tooth it is that is bothering you. Anyways, I personally hate root canal retreatment. If the root canal did not work the first time, why should it work the second time? There are exceptions of course, but generally a root canal retreatment does not clear the tooth from infection (exceptions are one or two nerve teeth where the root canal was performed below the standard of care and there is no infection on the tooth). Having said that, there is a good chance that your tooth is still infected and the root canal is only masking the symptoms. The only way to tell if this is the case or not is a CBCT of the tooth. The infection could be hiding behind the tooth or in between the nerve, and that only shows when you take a CT scan. I just don't understand why people insists on spending so much time and money on extending the life of an infected tooth when they have the option of removing the tooth and replacing it with the implant? This goes for patients, but more so for the dentists. I hope this helps. Feel free to send me an X-ray of the tooth and I will evaluate it more carefully. good luck either way!

He’s correct,corporations are making implants a big business bc it’s near 5 billion in 2024 and will be 7.4 billion in 2027 in USA alone. Best to seek dentist that owns the practice- but many still price gauge & fail to possess the SKILL & tech needed to do best job bc u do not want to pay again for failed implants- more common now due to high demand.

Holistic dentists claim to use only natural ingredients. But many infections cannot be cured with tea, tree or clove oil. DNA & one’s immune system determines if I holistic will even work effectively.. 80% of the population grind or clinch unknowingly- so a crown with no metal will crack easily. DIET- huge factor in oral health.; eating hard foods, sugary drinks & foods, acidic foods/ fruits and drinks, even 1 nut can chip or crack a tooth/molar, especially as one ages. Also, even if born with great DNA ORAL ENZYMES, medical treatment/ accident can adversely affect one’s oral health, especially saliva which carries the good flora to protect against bacteria and entire mouth. If one is in INDIA or Malaysia, they have done tree that naturally cleans teeth IF one is young, healthy & has no sugar diet, eating like natives. Not ALL natives have great teeth- they have dentists there too- it’s usually a lot cheaper than USA. Austria has great national health system that includes dentistry. Some USA ppl travel there bc got family/ friends to stay while getting implants / more costly procedures., paying all cash.

Implants in 2024 is 5 Billion $$ business in USA& insurance companies state implants costs will increase 2.3 Billion each yr forward, to 7.4 Billion by 2027. You are kind to explain these costs, but in truth few insurance covers most dentistry procedures despite demand & new costly tech . I know all the new tech has monthly maintenance paymnts too, like most software. At least u have real experience- so many get botched jobs bc ppl cannot afford over $10k for 1 implant by specialists.

Thanks for the information doctor. I got into a bike accident and my two front teeth were broken at gumline. It's so heartbreaking because they were healthy and now I have to get a rct. Is there another way of saving my teeth?

Hi there. I'm sorry to hear about your accident. If your front two teeth have broken at the gumline, then the only way to save them is with a root canal, post and crown. You still need some tooth structure above the gumline for this solution to work long term. Otherwise, if it's really broken at or below the gumline, then removing the teeth and dentures or implants is the better choice. Here are a few tips if you really want to save these teeth: 1. Consider connecting the two crowns: If there is hardly any tooth structure left, then the crowns may fall off. Connecting the crowns improves their longevity. 2. Crown lengthening: If there is almost no tooth structure above the gumline, crown lengthening can extend the useable tooth structure to improve the chances of your crowns staying on. These treatments are not cheap. Make sure that the teeth have a pretty good prognosis with root canals and crowns. Otherwise, remove the teeth and replace them with a denture or implants. So the most important factor here is how much tooth structure is left above the gumline. The more, the merrier! Feel free to send me your Xray if you want me to look more carefully at your situation. if the implant is done by cbct guided imagery thing is the implant hitting the nerve not an issue?

Hi Carl. It is very rare to hit the nerve during implant surgery. Using a CBCT guide is helpful to position the implants. It reduces the chances of hitting the nerve, but it does not completely eliminate the risk. Keep in mind that the nerve is only an issue for dental implants placed in the lower posterior region. The good new is, if the implant that has hit the nerve is removed right away, the nerve will mostly regenerate itself. How can you tell if this has happened? If you receive a dental implant in the lower posterior, and the numbness does not wear off after 24 hours, then the implant may have hit the nerve. This numbness could also be from the needle or the drill, but I would rule out the implant sitting on the nerve immediately. Contact your dentist and have them take a CT scan to see if the implant is on the nerve. If it is, have it removed right away. Otherwise you could be looking at long term, or even permanent, ,numbness of the jaws!
