As a fit 45-year-old grappling with left knee problems since October 2022, my ability to run/play football has been severely compromised. Despite not worsening since its sudden onset, my condition hasn’t improved despite extensive interventions, including physiotherapy, steroid and hyaluronic acid injections, and two PRP injections. MRI analyses from February 2023 and September 2023 have shown consistent findings: 1. Osteochondral Damage: Remains substantial in the patella area. 2. Bony Edema: A pronounced bone reaction with subarticular bony edema and cystic changes has persisted. I also have Quad Atrophy. Your expertise in patellofemoral conditions prompts me to seek your advice on potential treatment approaches that could help manage my condition. What initial steps would you recommend to alleviate pain and enable me to return to playing football for my over-35s team, aiming for pain-free performance during and after play?
