Ok treat it as kaioken and ssj3 they use up alot of energy and puts a strain on the body while using it and after so the stronger you get and train your body to handle it the longer and more times you can use it

8 gates, and the 8th one is called Death Gate. you can use 1st-7th gate everytime, well not everytime. the effect after 6/7 gate maybe kinda hurt so you need some time to recover. but 8th gate is use all chakra to your heart, even the punch itself gives an effect like your bones broken. that's why it called Death Gate, because nothing left to your body if you use everything while activate it

Yes. They also made a big deal about Kakashi being tired very quickly and low on chakra in the first 50 or so chapters of the manga. And then towards the end of the manga, Kakashi repeatedly uses the mangekyou sharingan. Surprise! Characters do grow within the manga. @@Tomouryu

To be fair, Guy is like the ultimate foil for Kisame. Even without Samehada most of his jutsus absorbed chakra and Guy's techniques are mostly taijutsu.

If a host dies, the tailed beast (or its chakra) is released, but it takes some time till it can manifest itself again. That’s how it works if I remember correctly.
