VY2 actually doesn't have an official avatar, unlike most other Vocaloids. His boxart is just a wakizashi (type of sword) to allow for more creative freedom. All the pictures you've seen of him are what artists think he looks like.

Other VY2 songs (originals or covers) usually quite realistic, like Error, Rolling Girl, Kidzuite Yo, etc. That's why I like VY2.

emily had answer your first question, and I will answer the second one. Actually the "P" is the short form of "Producer". Usually people who compose, arrange, tune or write lyrics for VOCALOID songs will add a "P" after his name. Some producer use two names, such as Kurousa-P. He also use the name WhiteFlame.
