They are solar comedowns sadly this elderly gentleman will have them for the rest of his life. All she can do is relieve the itch they cause atm. Most have them extracted each year. Trying to get everything out on his frail skin would cause more damage.

Ofcourse some people doesn't know they have to do with their facial skin tho, mostly people who are isolated from modern society. They never thought about facial hygiene and beauty standards.
