I love how they drew Astolfo with a somewhat manly body proportions (small hips, straight lines from armpit downwards, etc). He's a lanky guy that can fool people with appropriate clothing, not necessarily "a girl with a dick" that a lot of titles would use as a shortcut to draw their traps.

Rulers can see any Servant's character card, but Astolfo vandalized his by scribbling "Le Secret❤" over the gender entry. Neither Jeanne nor any other Ruler can uncover the secret by looking at his character card, and they have to figure it out either the old fashioned way (as Jeanne did here) or through other means. Strangely, though, everyone else can figure out that Astolfo is a guy at just a glance -- Masters, other Servants, and even random passersby, don't need to check to know he has a 🍆 and not a 🌮. This is why Gudao was confused that Jeanne had only just found out.

No he can't... Servants are summoned the way the Grail decides best represents them. If you're referring to da Vinci having the appearance of the Mona Lisa, that's a special case where the Servant was skilled enough to alter their own Saint Graph to be summoned with a different appearance.

which makes little sense since as a Ruler class servant she knows the true identity of each servant in the war, she should know he is a guy, if that is Jeanne, Jeanne knows how rare it was for her to be on the battlefield, so it should be understandable Astolfo is a guy if he served under Charlemagne. If it is Laeticia then it makes more a little sense since Laeticia is just a young girl and doesn't know his true identity as a servant but also not, since Laeticia is also french, so she should be aware of Charlemagne and his 12 paladins she should also know Astolfo is a guy, granted he is one of the lesser well known ones, but in the Fate universe everyone assumes Artoria and Mordred are male because they are English knights and that's how history remembers them in that universe, it would be fair to assume french knights should also be male.
