this was so useful! I just started my neonatology rotation and all the fluids calculations confused me a lot, especially since I'm bad at anything involving math. But this made it so much easier and even fun to practice. thank you so much for making this great video💓

Abeer! I'm going to stop you right there! YOU ARE NOT BAD AT MATHS!!! To get to where you are means you have an excellent understanding of numbers and maths. Every time you say that to yourself, you will believe it more! Somebody must have told you that when you were young and now it's become a fact in your mind!!! So from now on- think- "I've got this, it's easy" (because when you understand it- it really IS easy!!). Thank you so much for writing in and watching these videos! Let me know how your studies go!!!

I would love equations and tips/tricks for ETT depth insertion, UVC/UAC insertion if you had those up your sleeve. This one was a brilliant one too! Your channel and SNL are the only reasons I come to youtube!

Hey Tala, I am new to NICU nursing and was recommended your channel. I have been binge watching since! You are awesome. I just have one question. I cannot figure out how you got 80mL/Kg/day on a 1.2k baby when the 3kg baby gets 72mL/Kg/day. Did I miss something?

Thanks so much for the lovely compliments!!!! So I was just making the point that in little babies I'll multiply by 3 and then round up. So instead of running at 3.6ml/hr (1.2 X3) I'd randomly round up to 4ml/hr. This would give more than 72ml/kg/day which we would want in preemies. In full term babies we'd want to round down. (Just to emphasize, this is just to get something running until somebody sits down and calculates exactly what is needed!) So glad you're here :))

Thank you so much for these videos they are so helpful! I just had one quick question I’m not sure how to know that rounding it up to 4 is about 80 ml/kg/ day? Or that rounding it down to 3 is about 60 ml/kg/day of fluids, I did understand why multiplying it times 3 is about 72 ml ( because you multiplied 24 hrs in a day x3 and that gives 72ml) but when I try doing it with the other values it doesn’t come out to the numbers you had, I’m so sorry if I missed something in the video, I’m just confused with that part. I really appreciate in advanced your help!

HA excellent point Rebeca!!! We were just trying to make the point that it's a really ROUGH calculation to start something / anything! until you get around to doing the exact calculation you want. So if it's a micropreemie- round up- or even randomly add 0.5ml/hr, and if it's a full-term baby, round down or subtract an ml/hr or so. Sorry if that was confusing!!!!

For admission fluids we’re often starting between 60-80ml/kg/day - since there are 24 hrs in a day, 3x24=72 will give you a number close to preferred initial fluids.

@TalaTalksNICU thank you so very much, total fluids intake on the baby weighing 3.9kg,and I have decided to give baby both expressed breast milk , and 10% dextrose. ,much is going to be my oral feeds and how is going to be my fluids per Hour,please help I had misunderstanding with my senior According to my calcification was 3.9kg×60mls=234 total fluids intake and I divide 234 by 3hrly feeds which gave me 29mls every 3hrs feeds was I right,?

Hi mam am an engineering student and we got project to build any prototype which will help in NICU.so we came through this "Design and develop a syringe with integrated sensors and technology that can accurately calculate and display the amount of feed for a baby. The syringe should consider factors such as the baby's age and nutritional needs to provide precise measurements. The goal is to simplify the feeding process for caregivers and ensure accurate and efficient feeding for the baby." What is ur opinion about this idea mam .

Hello!!!! I’m so happy that you are doing this! What a great project! Honestly we are so used to calculating every day how much a baby should eat/ calories etc that I don’t think that calculation would help me much. Maybe a feeding tube that changes color when hits the pH of stomach. Or a tube that slows down feel if milk sitting in stomach (so feed would be over say 90 minutes instead of 60 to allow milk to leave stomach). Or some sort of tube that allows aeration of stomach while baby on Cpap but also feed at same time?!!! Lots of issues we have with feeds!!! Good luck!

Hi dr, if baby is on TPN, with npo, dr decide to start Feeds, is it ok to adjust Tpn rate from TFI?

Hello! Great question! It really depends how much you’re changing the feeds. If only going up a little then generally the provider will account for feeds with new bag of TPN. But if starting back at half volume feeds or whatever then we’d probably want to adjust rate.

generally- on thefirst day infants are given anywhere between 10-30ml/kg, and on the second day- between 10-50ml/kg. This is a wide variation- so I just do the maths! Sorry

Hello Nicole- this is a good question! In many units in the US- we generally increase the total feed volume up to about 150-160ml/kg/day. (Higher side if enteral, lower side if IV). At this volume, we can normally give the calories a baby needs to grow (between 20-26kcal/kg/day depending on the baby)
