This is a nice demonstration of a technique to run larger vessel repairs. I do not perform vascular repairs on large vessels such as the aorta, so my observations on technical aspects when doing microsurgical anastomosis on smaller vessels may not apply. When doing small vessel work one should never grasp the intima of the blood vessel with a forcep as this causes damage that can lead to thrombus. Also when working on small vessels the needle holder should be released before the needle is inserted into delicate tissue as the act of unlocking the needle holder can tear or damage the vessel wall; similarly the needle should never be held by two instruments simultaneously when it is in tissue as this increase the risk of tearing or damaging delicate tissue.
September 24, 1988 7:30 AM. I was 16. A metal chip from a splitting wedge lacerated my right superficial femoral artery. A parabola 20-30 feet splashed in the grass. Brightest red imaginable in the sunlight. Suddenly relaxed feeling. Put my thumb on it. It sprayed around. Pushed harder. Load back on heart. Ran 200 feet to house. After fighting off incredible urge to sleep and vomit, drove to ER and literally walked in. 18 hours later and three hospitals later it was repaired via anastomosis. Leg was 31 inches. I am eternally grateful to Dr. J. The flashbacks are always there though.
How could an aspiring medic learn this skill? I think it would be good to know if there was a situation where I needed to close up an artery. Also, what equipment is needed to do this proceedure? Can it be bought online?
I see you passing sutures ( foreign object) through the walls of a vessel, meaning traversing into The lumen, but this is prone to thromboembolism thereby needing some extents of anticoagulants. From what I know, a vascular sutures is not supposed to transverse into The lumen. It's supposed to pick only the side walls of the vessel thereby avoiding The lumen.
But sir, when you put cotinuos sutures the problem is that when the clamps are released the sudden lncrease of pressure of the blood flow will make the small gaps between the continuos suture for leak of blood, and all the more you will also be using heparin post op, And the bleeding can increase. Well if the sutures are tight then it's okay. As otherwise it is better to put lnterupted sutures for main vessels. My personal view. Dr. Kerala. South lndia.

sorry to say this, but this is a very rough technique in demonstrating a surgical technique. you don't wear a glove, your ring is still in finger and the way you are handling instrument is poor. the junior doctor will find difficulty in learning something that is contradicting to a principle of surgery
