This week at Grand Rounds we're joined by Pascal Scanlan (again!) and colleagues Jill Lowell and Michelle Logan.
Medicines Reconciliation led by pharmacy teams is now embedded in all GP practices following receipt of Immediate Discharge Letters and Out-patient communication. This service has been identified as one which could be delivered more efficiently. One of the aims of the revised GP contract Memorandum of Understanding (MOU-2) is to have appropriate activities led by pharmacy technicians. If the information received into the GP practice was in the NHS Tayside agreed communication formats, followed agreed processes and contained high quality information then this activity should be more manageable by technicians. Dr Pascal Scanlan will discuss the collaborative work he is undertaking with the pharmacy teams working with secondary care services to improve the Prescribing between Secondary and Primary Care.
