topical finasteride 0,025% is the closest we have at the moment with only 35% systemic DHT reduction, pyrilutamide might be worth checking out as well but the safety is not very well documented

Finasteride never " stops working " . You would always be better off while on finasteride than without finasteride anyway (even if you start losing a bit of hair few years after starting finasteride). What I did personally to be sure I won't lose anymore ground in the next 30 years is : I added dutasteride at 0.5mg twice a week (on Sunday and Wednesday) and I take finasteride at 1mg on the other day ! 👍. while yes its true that finasteride doesnt stop working theres no proof that we get more senstive to dht as we get older, over 92% of people who saw results on finasteride in the first year they had better hair at 5 year mark and 10 year mark on finasteride compared to 1 year. For the few that are more sensitive and finasteride is not enough what is happening is that the dht hitting the hair over time makes an acumulatory effect as the hair gets thinner and thinner.

@akilbarboni4198 fair enough !!! I actually forgot that fact (it's from the Japanese study if my memory is correct). I still think that adding a bit of dutasteride on top of finasteride can be the " little boost " you need to actually get more " regrowth " especially for people like me who got into the game pretty late (11 years on minoxidil only until i was Norwood 3.5 without taking any finasteride) ... big mistake but whatever .. I'm trying to go back in time basically and trying to get as much regrowth as possible to avoid a second hairtransplant (already had one). My stack is : - 1mg finasteride 5 times a week. - 0.5mg dutasteride twice a week. - minoxidil once a day. - microneedling at 1.25mm every other week . My goal : regrowth ! (Not just maintenance).

@@exactly1176 oh absolutely dutasteride 1-2 times per week is a sweet spot u have a great point there. Though I get why u do microneedling every two weeks to let the scalp heal, micro-needling on itself aint gonna do much if nothing sorry to break it to u, though theres a great place for micro-needling if u are using min, but I d highly advice to use it weekly at 1.25-1.5mm to since copled with minoxidil thats a 4x jump from just minoxidil alone according to a good study. My point being micro-needling that infriquently wont make minoxidil work on its true potenticial of u want to maximise ur regrowth 1x week is mandatory. and for me was a game changer, btw how are ur results so far?

My hair is falling out in clumps during the shower, while blow-drying it, and even when I come to style it a moment later, i get strands of hairs in my hands.I have been shedding for years but no sings of improvement while being on treatment. My hair is worse than 10 months ago. It's really confusing. Maybe finasteride is not meant to me?

@akilbarboni4198 actually I'm aware of that. I use topical minoxidil by the way . And I've been doing microneedling (with a dermastamp) at 1.5mm for 3 months exactly (did it once a week at 1.5 for 3 months) . It's only been a month since i started going for 1.25mm every other week. Why am I doing that ? Cause i think Kevin HairCafe got a point : We don't know what are the long term side effect of continued use of microneedling.... (we have no " long terme studies " on microneedling) we have short term studies but not studies for like 2 or 3 years etc... The question is : could " long term use of microneedling " could actually make our hairloss worse by causing scaring and fibrosis ?? So my objective is actually to stop microneedling when I hit the 6 months mark on microneedling. Of course I won't ever stop minoxidil / finasteride / dutasteride. My only goal with microneedling was just to get some more regrowth. I've never wanted to use microneedling for more than 6 months anyway as I believe the minoxidil + finasteride + dutasteride will hold on those microneedling regrowth. And as I believe (might be wrong) but I believe that microneedling might not be safe long term.

Hi matt, Good video but there is some misinformation. I read the 2 studies on topical dutesteride with microneedling. The studies apply mironeedling right after instead of waiting 24 hours! Moreover, Dr Mwamba, a reputable hair transplant doctor in Brussels, advices this same protocol.

First of all, thank you for your interesting and very informative and extensive program, I would like to ask a question: does it make sense to combine fin and dut, if so, in what ratio would it be advisable, or is the risk of side effects with the combination too high , after 8 years Fin it lost its effect and I switched to Dut with good success, thanks for the answer in advance!

For someone who (for a year) has been on oral Dutasteride at .5mg/day and oral Minoxidil at 5mg/day, and still losing ground fast with normal bloodwork, would you suggest 1.) Switching to topical Fin? 2.) Switching to topical Dut? 3.) switching to oral Propecia (brand name) at 1mg/day (brand Avodart is too expensive)?

@@MyPlanet123 I supplement only things that are low according to my bloodwork (vitamin D). Otherwise, all of my bloodwork is normal so no need to over-supplement.

Was good on finasteride until after COVID. COVID accelerated my hair loss. Now I’ve been on dutasteride and oral minoxidil for 5 months. Brought a good amount back but still some room to grow. Should I consider PRP on top of that stack?

Sam, COVID definitely has had a lot of negative impact on hair. But the primary reason why you didn't find finasteride effective post COVID, looks most likely that you must have developed tolerance to the medicine. Be careful, as the same phenomenon of tolerance could happen with dutasteride as well. Try to use minimum resources so that you can prolong your hair restoration journey. When it comes to hair, always remember, LESS IS MORE!!!! 👍👍

@@sakshiagrawal501 nah I don’t think so but it is possible. I think getting really really sick can trigger a major immune response, which can not only lead to telogen effluvium but also accelerated hair loss due to the immune response and inflammation. Fin/Dut lose effect over time more likely because hair follicles on the scalp become more sensitive to hair. But thanks for your insight.

I have been on topical finasteride for 1.5 years, no side effects but still receding more in my hairline. So now have been taking 1mg oral finasteride everyday for 2 months. yet i'm still losing a shit ton of everyday and my hairline keeps receding more and more, my hair is also getting thinner. Would dutasteride be the next best option?

oral fin shed cannot cause baldness even if you shed on it , its a good sign or neutral like in first 3 to 6 months of treatment there's no way it causes more receading , temporarily maybe

Yes, Switch to dutasteride 0.5 mg a day. Dutasteride has better results than the high dose of 5mg finasteride a day.

I use Fin (1mg/day) and minox (twice a day) for over more than 8y, I feel after 5 years I still keep loosing baseline. Now I will try oral minox and maybe topical dut. Lets see = trial and error 😆 BTW: Thanks for video, I thought that only I had this problem.

It's a great choice that you have made by starting finasteride in low doses rather than full dose of 1mg/day. Even microdose of finasteride is sufficient in arresting your thinning and protecting your existing hair. You are bound to develop tolerance to whatever dose of finasteride you are taking. So keep it as minimum as possible. That way you can make it work for quite a long time👍👍

@@PeteFarce it’s testosterone not DHT. The problem is you have so much left over free testosterone that can be converted to dihydrotestosterone but the Finasteride keeps the hair loss at bay.

@@ozmo69 lowered the TRT dose slightly, stayed on 1mg of Finasteride. Hair still looks good, maybe not as good as my profile photo but it’s good enough.

After taking finasteride for 10 years with amazing results, it suddenly stopped working for me. I’m 34 now and I am still taking finasteride but my hair loss is progressing rapidly. I am loosing 150 hairs a day. The hairs fall and never comeback this time. I’m on 1 milligram finasteride, 2.5 minoxidil. Topical 5% minoxidil and 0.3 finasteride. ( I took dutasteride 0.5 once a week , plus finasteride 1% for a year. I stoped taking dustasterude as I did not see any major changes) I tried everything from ru5 to natural remedies but nothing is slowing down my hair loss anymore. I still have a lot of hair but the hair line it is looking pretty bad and I am getting tired of hiding it. I might have to shave. Does anyone know of any solution to finasteride not working anymore?

Unfortunately Dutasteride was worse for me than Finasteride. I have been on Finasteride for 10 years and seemed to be slowly losing ground. Thinking that Finasteride wasn't strong enough for me I tried taking Dutasteride .5mg daily for 8 months and started extreme shedding from 1 month all the way to 8 months when I finally decided to stop because it was giving me so much anxiety seeing my entire scalp thin so much. The entire 8 months I was still taking 1mg Finesteride. 30 days after stopping Dutasteride the shedding slowed dramatically. I stayed on Finasteride for another year and thought maybe I didn't give Dutasteride long enough and started again, and I also added 5% topical Ru thinking that would help, but I experienced the exact same non stop shedding. This time I quit at month 10 because my hair was just jumping out of my scalp. Im now adding topical Finasteride in addition to my 1mg oral Fin and shedding has stopped. Im not sure why my hair had such a bad reaction to Dutasteride both times I tried.

Almost all minoxidil studies on micro-needling have a 24h wait. I got influenced by those partly + You may get more systemic absorption from dutasteride if you apply right away. Also applying topical right after needling with 1.5mm - 2.5mm may not be pleasant as this needle depth is pretty decent already and immediate application can result in stinging or burning scalp sensations. I rather show an approach that is a bit more conservative because I know that some people may follow it and try it themselves. If you feel like you can tolerate immediate dutasteride application, then go for it.

Switched to dutasteride every other day and it worked much better than finasteride. Dutasteride every day will be the most effective and you just need to find a dermatologist willing to prescribe it or you can get it from Strut very easily. How was PRP? Thinking of doing that

I'm searching a Dermatologist that will give me that, Where are you living? and you know some Dermatologist that give it? I thinking to fly to other country to get a presciption. PRP don't help me. I trrying oral minoxidil 2.5 MG, PRP, Minoxodil foam, Propecia and more, and starting in Norwad 1 without any shedding and nothing helps me

i mean is worth the shot since u still gonna have improvements regardless as years go by. If u are having good results with fin in the first year theres extremely high changes acording to 10 year old studies that u keep having progress not just manitance.

1mg is the recommended dose. I feel you should add 1mg thrice a week and then eventually up the dose to 1mg all days of the week

Its really sad I have tried fin again after many months of not using it and within a day I have the usual decrease in erection quality and I cant hold it up at all, it vanishes within 3 seconds or so... also less penile sensitivity. Pls god give me something else.

i thought i had that too, it was just a bad life style bad sleep and other stuff. Chances of having side effecs are low is far higher of a chance that is just from smth else. I have no issues now.

Hello Kevin. Please dont be so harsh on yourself. It's a very well documented side effect of DHT blockers, finasteride and dutasteride. There are other options like oral minoxidil, topical finasteride and also natural oral DHT blockers like saw palmito etc. Consult a hair restoration doctor, who is well versed in both surgical and medical techniques of hair restoration.

Dutasteride was worse for me than Finasteride. Only way I was able to slow my shedding was to start topical finasteride in addition to the oral finasteride that I have been on for 10 years. No new hairs yet (only been on topical 3 months) but I'm just happy with keeping what I have.

@@simonecusumano3170Have you lost ground in those 8 years with Dutasteride since 18? I started balding at 19, started Fin at 22.5 and Dut at 23.5. I regret a lot of hair and completely stopped the hairloss. Hoping what I lost in those few years of delaying treatment was going to go anyways by 40 or 50 years of age even whilst on Dutasteride. I started very early still at 22/23.

I‘m also using Minoxidil 10% and RU58841 but without results.. I never tried Finasteride as my dermatologist prescribed me directly Dutasteride as an I was 18.

Your body gets used to the medicine over long periods of time. Best thing is to give it a break for about two months and let your system reset. It worked for me.

Yeah I don't think this is correct. It's simply your hair follicles deteriorating as you age meaning they are more prone to miniaturisation rather than your body getting used to the drug over prolonged use. I could be wrong but I think that's what doctors say

For someone who is experiencing aggressive hairloss besides taking a safe low does finasteride orally can use it topically as well .I mostly think just using it either just orally or just topically is enough for 5 AR inhibition .I think for the best results of getting the most out of your finasteride working pair it up with using an anti androgen blocker in your topical .whether it be RU58841, pyralutimide or brezula .note if using brezula or pyralutimide as your anti androgen .buy only enough for a 6 month period use it up and switch to the other as those two anti androgen may only be most effective in a six month window

Finasteride never stops working , it always works. It's just that as you get older : your scalp becomes more and more sensitive to the little DHT still going throught it (not blocked by the finasteride) and the hairloss comes back but you are still better off than if you don't take finasteride at all anyway.

@EmotionalKatZ your very welcome. Don't hesitate to add once or twice a week dutasteride at 0.5mg on top of your daily finasteride if you ever start losing ground again on finasteride. It should be the extra little boost you need to keep your hair 10 or 20 or even 30 more years

@JavierPwns those people in the study are Japanese. Less prone to agressive hairloss imo. But yeah you got a point ! Still, usually hair loss does accelerate between 30 and 40 years old (for most people). That said : it's true that for most people, finasteride is gonna be working forever mostlikely, people will most of the time hold ground on finasteride for even 20 years or more. Finasteride is indeed very very effective 👌

Im looking in Dutch, if your girl is pregnant use a condom it’s not good for your kid. So I think the sperm is also not good for your girl to swallow 😅. But in Dutch they also say, swollen lips and breathing problems and painful nipples and more. The problems with this pills is not good, just like the other one. I had HT in the Netherlands and they did my front. But my crown is also a little thinner. But I’m using minoxidil for 3 months now and my other hair is so thick and better. Now I’m Derma rolling 1 time a week with 0.5mm and 1 time a week with 0.25mm and after that directly minoxidil. I do it in the evening and I don’t have any side effects. I will keep doing this and if my crown will be thinning I will get a other HT. That’s more easy to do, cost some money but no more things to take and bad pills. Pills are not good for your body

@luqmanmalek288 get on dutasteride 0.5mg every day and also add topical minoxidil 5% once a day. You should be alright with that stack. Edit : and also add topical pyrilutamide to your stack as soon as it's FDA approved 👍

I was on 1mg fin propecia for from mid 20s to mid 30s and it stopped my hair loss in it's tracks, I then took a 3 year break because I had mood issues and I had significant diffuse thinning so I went back on 0.5mg and its efficacy was poor, hair loss progressed. Seems my androgen activity has upregulated with age. Now I use topical pyrilutamide but I've yet to see any results hence I'm going to try dutasteride 0.01% mesotherapy at a clinic 1x month with microneedling dutasteride 0.01% at home for the weeks in between clinic visits. Do you know why we have to wait 12-24 hr after microneedling before applying topical dutasteride? Wouldn't this mean all the holes in the scalp scab up preventing dutasteride absorption? Be interested to hear your opinion on this. Thanks

Almost all minoxidil studies on micro-needling have a 24h wait. I got influenced by those partly + You may get more systemic absorption from dutasteride if you apply right away. Also applying topical right after needling with 1.5mm - 2.5mm may not be pleasant as this needle depth is pretty decent already and immediate application can result in stinging or burning scalp sensations. I rather show an approach that is a bit more conservative because I know that some people may follow it and try it themselves. If you feel like you can tolerate immediate dutasteride application, then go for it.

The reason why 0.5mg finasteride was not effective in your case was because you took 1 mg / day finasteride for almost a year. You can't downgrade doses of medicines like finasteride, as they are habit forming dependent medicines. It's better to start at a lower dose and titrate it according to the response you get. You will be surprised to know that microdosing of finasteride as low as 1.5mg/ week is quite effective in sustaining and improving your existing hair. The reason why transplant surgeons give it in full dose is mainly to show you flashy results, and justify their unrealistic number of grafts. The reason why as a dermatologist and a hair transplant surgeon, I wish people should refrain from using such high doses of finasteride is because of tolerance, dependence and of course side effects. These can be produced later on as well, like gynecomastia, depression episodes, etc.

@@user-bj9zs5iu6u It just started 2 days ago...But i am little better already.Drinking plenty of camomile tea and taking Valerian pill helped a lot.Also training and distracting your brain with something.Just 4 doses of 0.5mg caused that can you imagine? Don't take it if you did not start yet. Everyone is different, but for me it does not worth the risk!

Dht blocking medicines does accentuate the results by many folds, but if they are not working, or if the patient does not wish to take them, then there are other alternatives that one should definitely try out.

The most important thing for N6/7 is to have a realistic expectation from medicines and transplant. If you are planning a hair transplant, go for a slightly upper hairline, choose a clinic well versed in both FUT and FUE for maximum donor utilisation and above all look for realistic sustainable results rather than flashy short term results. Remeber your donor is limited and very precious, so use it well!!! All the best👍👍

Start finasteride/dutasteride, minoxidil, and weekly microneedling with a dermapen at 1.5mm depth. Do that for a year, see how much regrowth you get then plan a hair transplant

I started taking finasteride, in the first two weeks I had testicles, but I knew that this should pass after a while, everything was fine with libido, there was an itch in the nipples, the nipples were swollen ... I quit ... it's a pity, so I hoped to return my hair to life ... has anyone experienced swelling of the nipples, how to get rid of it? Give topical finasteride a shot mate ! Or maybe 0.25mg oral finasteride every other day + topical minoxidil. You've got this ! 👍 Just lower the dose (it still does work this way, and the side effects will go away on their own with long term continued use of the drug most of the time). Power throught it and don't give up. Hope it helps 🙏

@@elmarlion уверен?) есть личный опыт? Не охото рисковать, тем более раз уже появляются признаки набухания сосков и зуда в них. Правда страшно. Много комментариев читал о том, что кто-то так и не возвращал прежнюю форму груди. И бросать эту битву за волосы не охото...буду наверное сегодня мешать миноксидил с финастеридом и на голову наносить, внутрь принимать боязно.

@@laimyr57 У меня был зуд и он прошел. Самое главно следить чтобы не рос lump изнутри. Мне от зуда тогда ещё помог витамин B6 вроде. Но не уверен что именно он помог. Зуд начался на третий день после приёма 1мг ежедневно. Затем я уменьшил до 0,5 мг ежедневно и на 3 месяце все начало проходить. Сейчас спустя год решил принимать по очереди 1мг и 0,5мг. Либидо очень высокое, никаких побочных эффектов нет. Советую еще просмотреть канал hair cafe. Удачи 👍
