This video will show you how to replace the front output shaft seal on a 3rd Gen 4Runner (1996-2002) transfer case. After 250K miles or more, these may begin to leak and its a fairly simple fix as long as you have a few basic tools and know-how. The process should be pretty much the same on the rear output shaft seal, but I'd encourage you to hop over to Timmy the Toolman's channel to get the specifics.

Does fluid come out when you disconnect the shaft? Do you have to refill the fluid after?
@TheOutboundLane

Some fluid will likely come out and you should make sure it's topped off at the correct level when you are done. You can try parking so the vehicle is at an angle and that should minimize and leaking
