When he see that Tengen always appeared from the right side of Tanjiro to push him away by his "left" hand or shoulder out of Gyutaro's deadly strike like one aimed at Tanjiro's chin. That's why he decided to cut off Tengen's left hand and blind Tengen's left eye too. Because of losing left eyesight, Tengen couldn't fully protect tanjiro, so Gyutaro successfully struck a fatal hit with deadly poison at Tanjiro's chin as we've seen in anime. Theory 15 at 5 minutes. Manga did confirm at one point that Nezuko is child-like because for some reason she was pyschologically suppressed as her body was rejecting the demon blood. Even Ms. Tamaio mentioned her situation is unusual not because she doesn't kill humans, but because her child-like regression. When later she starts become sun immune she can finally speak because she is actually becoming more human.
