Usually we repeat blood culture after 48h from start of treatment or we decide to change selected antibiotics

Great question Andromeda- I guess the answer is that you need to repeat culture at SOME point if it is positive. When exactly may depend on the antibiotics you're on. So if you're on none- and you think it's a contaminant- you'd repeat before starting antibiotics. If it's positive and you're on a set of antibiotics, you may want to repeat before you add additional antibiotics. If baby is getting better, then maybe you don't want to repeat immediately. Does this make sense?

Great question- if the RBCs are elevated then the protein will also go up significantly (1.1 for every 1000 RBCs). Sometimes we see and inflammatory response in the CSF- so even though there isn’t an infection- the whole body has increased proteins. Generally though the younger the neonate, the higher the proteins we accept (even up to 150-200). Hope this is in any way helpful?
