Dr. Sanil Rege is a Consultant Psychiatrist and founder of Psych Scene (a platform to enhance psychiatry education) and Vita Healthcare, providing high-quality mental health care services to the public. He is a Fellow of the Royal Australian and New Zealand College of Psychiatrists and Member of the Royal College of Psychiatrists (UK). His clinical and research interests include psychosis, depression, anxiety, post-traumatic stress disorders, and personality disorders.
Dr. Sanil Rege begins with an overview of immune dysregulation in bipolar disorder and the role of cytokines in neuroinflammation.
Dr. Rege discusses neuropsychiatric systemic lupus erythematosus (NPSLE) and talks about the pathogenesis of the disease.
He covers the association of encephalopathy with synaptic antibodies and the management of synaptic encephalitis.
The next autoimmune disease covered is autoimmune thyroiditis and he discusses treatment using steroids, concluding the presentation with a list of symptoms and signs that should raise suspicion for autoimmunity.

Thanks for listening. I’m 4th generation . My Undiagnosed autoimmunity for the last 35 years with doctors barking up the wrong tree, finally has bubbled over into serious debilitating disease.

@@PsychiatrySimplified Sorry for not seeing your question sooner. There was no diagnosis just a shrug of shoulders and a “ your getting older” (at age 35) response to my statin induced Rhabdomyolysis and unchanged refractory cholesterolemia, with off the map Thyroid antibodies and four generation autosomal dominate inheritance patterns of all symptoms. CAD at 52 requiring cardiac stenting. At 63 3x bypass and tissue implant replacement valve. Post surgery respiratory failure tracheostomy and 14 days on life support. Upon resuscitation Tapia’s Syndrome required tube feeding for six months and now it’s five years later ( AND I AM ALIVE) with cranial nerve palsies and now my autoimmune thyroiditis has gone wild and TSH varies between .0007 and 55 on a week to week basis. I have severe “thyroid” eye disease with diplopia and the symptoms of RA, PMR, SEL, and MS and the antibodies to match thanks to my ophthalmologist Aaron Judd, OD I am now waiting for rheumatology consult in (6 weeks). He has been my Huckleberry.

Thanks for this information. Mom has schizophrenia and the antipsychotics have a very weak effect. She has auditory hallucinations. Used to have Seizures 20 years back. What can be done. Whats the solution? How to test for these autoimmune conditions and what is the treatment :(

It's not possible to provide specific advice as this requires a detailed evaluation before implementing a plan. A neuropsychiatric / neurologist evaluation to rule out or to identify the role of seizures would be helpful.

​@@PsychiatrySimplified Thanks Dr Sanil. Autoimmune Encephalitis Panel came negative but found Anti TPO antibodies (214 IU/mL with Positive Reference: >9). Mom has been on Levothyroxine for hypothyroidism since 2012. Mom had a benign meningioma tumor removed from CPA in the brain this year. Biopsy revealed Interspersed Lymphocytes. Her MRI shows Mild Diffuse Cerebral Atrophy along with White Matter Lesions (T2/FLAIR hyper-intensities in bilateral frontal periventricular region). Autoimmune/Keto Diet helps a lot when avoiding Wheat gluten, Dairy, egg, Sugar, etc. Her condition worsens on having those trigger foods. Meeting with a neurologist to see if IV Methyl prednisolone 1g x 5days shows improvement for Hashimoto Encephalopathy. Thought i should update this comment for you. Thanks!
