About Lulu and Nana: Twin Girls Born Healthy After Gene Surgery As Single-Cell Embryos

Two Chinese girls, who we'll call Lulu and Nana to protect their privacy, were born healthy a few weeks ago. Their mother Grace started her pregnancy by regular IVF with one difference: right after sending her husband's sperm into her eggs, an embryologist also sent in CRISPR/Cas9 protein and instructions to perform a gene surgery intended to protect the girls from future HIV infection. The surgery reproduces a natural genetic variation shared by more than 100 million people of primarily European origin that confers strong resistance to initial HIV-1 infection and disease progression. While CRISPR/Cas9 has been studied in human cells and in early clinical trials, gene surgery in embryos intended for pregnancy has not previously been reported. Safety remains a key concern, particularly for unintended changes to the genome. To assess the girls' genomes for safety after the surgery, multiple whole genome and targeted deep sequencing techniques were used before implantation, during the pregnancy, and now after birth. These data indicate the girls' genomes' were changed as intended by the gene surgery, but no off-target editing or large deletions occurred. Further assessments to confirm these findings will be conducted over the next year. We will publish our full data soon.

While I understand people who are scared of "designed babies" my main objection to their objections is logical. Few people today still object Transplantology despite obvious countless controversies, procedures that are by definition defying integrity of human beings often go even worse for patients not to mention fueling criminal activity and human trafficking! Personally I found therefore Germline CRISPR if carefully planed on well understood genes Far LESS controversial! Than already existing other medical technologies. And it's doubtlessly better to eliminate the cause of illnesses and sufferings than to ineptly treat their symptoms. He Jiankui is a real hero of Humanity The Gagarin of XXI Century! Pity that while Gagarin was worshipped by people throughout the world regardless of cold war affiliations Chinese Stasi got He arrested right after his conference were he presented flawless germ therapy results!

Because we have no idea how a person with a conditional 1000 iq will feel, will we be able to detect a new mental illness in them, if there is one? And in general, will he or she be a human being in the usual sense of the word? And there are many more factors that, at the very least, should make us take gene editing more seriously.

The reason why this topic isn't more talked about is probably because there is an actual lack of evidence of the experiment actually working and creating such a breakthrough, as claimed by the researchers. The research team was anonymous, the experimental process (reportedly) wasn't documented properly, and no external research committee has reviewed and verified the research process and its results.

Gene editing is one way to cure disease, but not at this stage. CRISPR-CAS9 is still premature with high risk, this is illegal in China and is a huge ethical issue in science world

Nidhogg I think the social impact will be more concerning. Rich people would pay big money to ensure their offsprings have a competitive edge, while average people remain “inferior”. This will create a bigger problem than income inequality, as people assume their future is hopeless and their children will be enslaved by a superior “species”. We still don’t know the long term effect, maybe With enough modified gene in the pool human will lost the ability to reproduce, and there’s no way you can stop it since people are paying so much for something that can be done easily at a local underground lab.

Nidhogg When you talk about humanity. Think again what is humanity. When the gene editing technology still has high risk and unpredictable outcomes, you can not use on human embryos to born a human. More serious issue is that if you editing the genes (one or amount of) you can not even define the baby is still homo sapiens or not, how can you speak humanity any more? This is ethics issue. Gene editing is not a normal technology as you mentioned plastic or rocket thing. Think Again,why this is not legal in US or China, if it’s better for your so called humanities.

Nidhogg Apparently, you’re still unclear about this technology and its risk. Gene editing is one way to cure disease for sure but not at this stage. This experiment is no different from what nazi did in German or China during WWII. Think this in a different way, if you want to have a baby, do you agree with perform this unpredictable surgery on him or her? Do you want to add the risk for cancer or other diseases for your baby? I believe you don’t want to do that anymore. See this called humanities, regardless you are a scientist or a religion one, you will make this decision as a human, nothing about evolution.

He failed to achieve his goal, neither of the babies is immune to HIV and both of them have a version of the CCR5 gene that has never before been seen that may compromise their ability to fight infection. He broke various Chinese laws (including gathering informed consent from the parents, you see he didn't inform them of the danger, and illegally practicing medicine) all to do something we already can do, prevent the passage of HIV to offspring

This actually could be fake, just to raise awareness for the controversy around human genome editing. There are some red flags: 1, There is no paper or preprint 2, Hospital doesn't know about the existence of the babies and 3, I fail to understand why you would knockout a receptor to which HIV binds if the dad is the one with the HIV. Isn't the HIV removed from the sperm when you do the IVF treatment? 4, Whole genome sequencing before and after editing is a very good idea, however it is very very hard to make 100% sure CRISPR did not aspecifically target something. Variant calling and other bam analyses would detect changes in the DNA if the depth is good enough, however if for some reason the CRISPR-cas9 creates or inserts DNA that does not align to the human genome, then it will go undetected with those analyses. You could write some scripts to check changes in the unmapped or align against other genomes like virusses and transposons that are not known transposable elements, but I doubt that they did that.

@@GM4ThePeople If I understand your point, you think that they want to engineer ppl to be gay? Idk if ppl want that, but too me that sounds really hard. They think homosexuality has more to do with epigenetics than with genetics, which makes it a lot harder to modify. But still, changing the whole mechanism of something is really hard. For example, what they claimed to have done here, is disabling one gene, which is easy. But when you want to change something like the way an eye functions or the when someone gets angry, there are so many processes involved. You would first need to map tens of genes involved in the cascade and then know how they work and know how you can change them for the better. It is soo many information and right now we do not even know what most genes exactly do, let alone in which cascade they belong and how they exactly function. Now because homosexuality is even more complicated than that, I do not think that we will live to see that ppl can engineer sexuality.

Dr. He claimed “before returning Lulu and Nana to Grace’s womb, we checked how the gene surgery went by whole genome sequencing. The results indicated the surgery worked safely, as intended. Grace’s pregnancy was normal, which we monitored closely by ultrasounds and blood tests. After birth, we again deep sequenced Lulu and Nana’s whole genomes. This verified the gene surgery worked safely: no gene was changed except the one to prevent HIV infection.” As far as I know (sorry, I am not a specialist in this field so I might have misunderstood) the whole genome sequencing to find one nucleotide change requires substantial numbers of cells to normalize the mis-reading of the sequence. How can he get such substantial number of embryonic cells before returning the embryo into mother’s body? And what was the "reference" genome to check if there are potential changes of the sequence?

During the period of incubation, do you think he maintained the embryo to put into the womb? Honestly, I think it is very unlikely. @@user-Chinano12345 我从来没有反对过gene editing, 甚至我实验室还在做cas9 delivery相关研究,可能并不触及gene editing本身,但也轮不到你一个把基因编辑等同于测序,从而觉得编辑后代就几百美金的人来指责我门外汉。制毒跟制药的技术基础还一模一样呢,是一回事吗?是的,测序不贵,全测序都商品化了你看学界有联名声讨吗?编辑一个后代可不是做几个测序就完了,你尊敬的贺老师做了近60页的ppt还没把这整个实验讲明白呢,你知道他花了多少钱,他又知道你已经替他制订了薄利多销的市场战略了吗?我也不是经济学专业人士,就是顺便修了个二学位罢了。我们来讲讲市场经济下现实的不公平,很多药物,比如抗癌药的价格就让很多家庭难以负担,而药的价格是由实际的生产成本决定的吗?不是,我们还要回收前面十几年的研发成本,而法律也必须予以支持,专利制度正是从市场角度保护创新力。所以无论是因为这项技术的灰色发展无法面向大众,又或者单纯从市场经济考虑,它必将在相当一段时期里受到某种程度的垄断,这期间造成的社会效益会有人为之买单吗?另外,我的观点也并非科幻小说里来,我甚至没有阅读过相关题材的小说。有关基因疗法的伦理问题有相当多的研究文献,内行人请多少看一眼。

@@user-Chinano12345 你想象中未来要做一个转基因后代就跟现在卖卖美瞳跟假发差不多,那天可能会来,但我们的担忧更多是着眼于眼下的。所有反对这个实验的科学家,不是在反对基因编辑本身,这一点你要认清。所有的争议在于此刻能不能、该不该进行活体实验。技术本身存在太多风险,我看过整个报告,实验设计上勉强能说尽可能降低了两个孩子身上脱靶的问题,但这不等同于这两个孩子及她们的后代(如果有的话)没有承担其它的风险。这个技术真正成熟还有很多要做的,而我们不觉得争个一朝一夕而让普通人去承担这些风险是科学进步的需要。你说我不懂经济不懂市场,我可能学得不精,但是经济学在我眼里是理解人性的学科,你说伦理是垃圾,因为人性本身就很肮脏,你说限制应用于显性的外部缺陷,这不就是一种约束?要么基于法律,要么基于道德,绝不可能基于市场经济。经济学的基础就是需求与供给,只要有人不再满足于平均的各项指征,就会产生需求,就是一种商机,除非你哪天把人类逐利的本能编辑一下,否则哪怕是黑市交易,都会有人去做,毕竟成本不高啊?

These experiments are highly controversial and we don't know the final results. As I know he is in prison at the present time in China due that he violated China's regulations. I think he tried to help science but did not realize that this technology may be used for different uses.

I don't understand what he did wrong, aside from practicing illegally. He made GMO children? Should be fine for people who approve of GMOs. He modified embryos? Pro-choice people dismiss potential for the future of that embryo in favor of the parent's choice. Therefore he did not "harm" the children, just as how you cannot "harm" the fetus by killing it. I don't get why people are being so hypocritical about this and saying it's wrong.

Perhaps the rush to be first is more short-sighted arrogance than anything else. Plenty of people know how to do this, including the people that invented it at Harvard, Berkley, and a handful of other labs including one in Shanghai. The issue wasn't that it couldn't be done, but should it be done. It is a very different thing playing with a child who has no say in how they will be born, and working with an adult that can give consent. Plus there is no consideration of the additional generations this experiment affects. There is so much we don't know about our DNA, and the collateral damage this technique may cause to other genes that are dormant, but we may need in human kind's future evolution.

So with the argument about "consent", should we stop circumcising babies? What about implementing religion or giving them a name they didn't choose? So, let's ignore that issue. We can never predict the effects of our actions. We didn't foresee the climate impact of industrialization, but you wouldn't be typing on a tiny computer that fits in your pocket if we didn't pursue it. So let's ignore "considering the future effects" cause we can't predict that. And technology naturally affects our species' evolution. Discovering fire and using it to cook your food, is the same technological advancement as using CRISPR. So let's ignore that argument too.

However, among scientists in the field we are concerned about long-term effects of gene-editing. We aren't sure if there are any unintended consequences of editing certain genes in or out or whether this technology is as precise as we wish it to be. Combining that with how much we don't understand about the whole system we're manipulating and it could be dangerous in the long run.

Thanks for your reply @@boukmann1791! I looked in multiple articles online and searched in published papers and apparently circumcision prevents UTI on babies. Circumcision is a good thing. The only bad thing is, kids don't have a say on the matter. It's like feeding your kids/babies vegetables. You know the benefits of it, but they dont. What do you say to that? And also, it's hard to predict the effects of our action. Did you know we don't know how analgesics work? Yet Tylenol is over the counter. Look it up! The effects of 5g is still being debated, and it's coming gonna be on your next mobile devices. We can halt technological progress until we're 100% sure it's only gonna be for good, but we will never move forward.

The funny thing is, that the guy KNEW about the papers on supermemory mice with the deleted CCR5 gene. The edit was made not only to make the children immune to HIV. It was partially done to make some sort of supermemory superhumans. xd

The Biggest mistake that He and his team made was that he went very public about his experiments. Given the ease of using CRISPR-cas9 technology, it's clear that there are many experiments already under way that we don't know about.

The ethical problem which arises with wanting to raise your Childs IQ via Gen editing is that you deem someone unworthy of living or at the least less valuable, if they do not fulfill your/societies expectations of intelligence. Why should it matter, if your child is of average or high intelligence. It should and cannot determine a persons worth. Having an high IQ does not automatically promise success, happiness, health or any other factor you would believe would come with above average cognitive abilities

@@waspoppin4784 Lol what? What he did WAS unethical in so many ways. First, he obtained false consent from the parents by deceiving them (told them it was some vaccine program). Also, he effectively did a procedure on humans that's barely been tried with animals; in medicine, you never try out a therapy on humans until you understand the safety, efficacy, and mechanism/rationale of the individual steps of the procedure. Also, even if gene editing was a common thing, there are far less risky procedures that would be tried before turning to editing of the embryo; for example, editing bone marrow stem cells in HIV infected patients so that you're not messing around with all the cells of a human body. Gene therapy holds enormous promise, but it has to be developed more responsibly and cautiously than this.

This is not new actually scientists trying to do this more than 10 years ago but the strict regulations require them to destroy the embryo after 14 days and nobody was allowed to use human cells to do experiments like this. But they actually started doin g animals a while ago

If you support this guy, then you clearly did not look at the full extent of the risks and implications of his experiment. 1) The experiment was not approved, first of all. 2) He did not inform the parents of the risks. 3) He created frameshift null mutations because the CCR5 alleles were not created in the twin girls. As a result of this, one of them is mosaic (this means that it was genetically different from the twins, which can cause disease in the future) and the girls may still susceptible to HIV. 4) The scientific basis for the study is weak: it is very rare for fathers to pass HIV down to daughters. It's difficult to say, but there might be other mutations and genes altered that might impact the girls in the future. Please be aware of the conflict in ethics and safety of this study.

Are the twins now immune to HIV? If so, can they pass those genes onto their children? If so, doesn't that mean eventually humanity will be immune to HIV if their genes manage to spread into future generations? Yes, this was so-called germline editing, so in principle the trait of immunity to HIV as we know it will be passed on to any offspring. Other than that, remember that the human species first has to manage to survive before you can get to your 30 generations. It's also possible that the HIV virus no longer exists by then.

Dude is crazy. One of the biggest problems with CRISPR is creating perfect guide RNA so that the CRISPR system doesnt run around trying to modify all DNA sequences that are slightly similar. Off targets are the biggest problem and could cause HUGE issues. This shit doesnt seem right to me

I can remember back to around the mid 90's when they first cloned the Sheep: "Dolly" Immediately after they were successful, two Italian doctors sought permission to clone Human Beings. They were refused. But who's to say that they didn't sneak off some place and do it anyway. Lulu has genetic mosaicism so she still produces some of the CCR5 protein. Jiankui progressed unsupported and I’m vanity for a disease that could be prevented through sperm screening, which he already did.

except that they may not have the immunity to HIV at all and also their CCR5 versions are possibly the only two versions in the world as they are made up but they are still able to be passed down by reproducing. Do you have any clue what this could mean? This is the greatest example to why we should be twice as cautious with this genetic engineering.

We must research and be wise to always consider science. I'm a biomedical student and I think if we can use science for the good, we are right. I respect all the religious and ethic's views, but if people think science is "playing God", we shoudn't ever have discovered peniciline. Think about it.

I know this is controversial and that using CRISPR to modify DNA has multiple side effects. However, i believe the intention of this project was just what he said: giving not-so-healthy parents the chance to have healthy babies. He said he checked the infants' genome after birth and the rest of their DNA was just as it should be. Of course, i would want to look at some actual scientific prove that he in fact modified their genome and the babies are HIV resistant. He could be making up the whole thing, but if he in fact succeeded with this process, i don't see anything wrong as long as the babies are healthy and as long as the process was the done with the consent of everybody involved.

To me, this man is a hero. If you want to achieve big scientific breakthroughs, you need to be bold. Sanctimonious bioethicists love to express "outrage and shock" at this type of work, but what have they done? They love to lecture, debate, and discuss ethical issues endlessly while admonishing people like He Jiankui for actually daring to get things done. Think about a world with less disease, longer lifespans, and smarter, happier people. If that strikes you as dystopian, then you're very small-minded. What's the matter, anyway? Are you just jealous that when this treatment becomes available to babies of the future, you won't be one of the recipients? Boohoo. Don't hold back human progress!

The problems being: one, there is no breakthrough and two, it’s not ethical. The technique has been used for years and close to being patented, just look it up. Nothing new was gained by applying it to human. What he did was imposing unknown and unnecessary risks on disadvantaged couples and their children.

Off target changes are so common and his lack of providing his research tells us he’s lying and this never happened or he made multiple off target alterations and fucked these little girls up for life and therefore future generations. I too want this sort of thing to take place, but he should’ve never done this in secret.

1. I’m angry within the limit of my ethical knowledge today, I suppose that I won’t be angry with it if I would have a “new” ethical mind. 2. He has not claimed any technological breakthroughs or changes. Assuming he is using the currently established technology for editing embryo genes, he would inevitably face the technical challenges that are currently plaguing the entire academic community. Firstly, there is the issue of "mosaicism." Using current gene editing tools, it is highly likely that they will not only work at the single-cell stage but also potentially at later stages such as the two-cell or four-cell stages, resulting in a baby with possible genetic "mosaicism." Normal individuals have only two genetic types, one from the father and one from the mother, while a "mosaic" baby may have three, four, or more genetic types. The effects of this on the two children are difficult to predict. What is a mosaic: An individual with different genetic traits or mixed expressions, also refers to one of the types of chromosomal abnormalities. Secondly, there is the "off-target" issue. Although off-target effects are not inevitable, they are also difficult to completely eliminate. The occurrence rate is related to the gene locus, cell type, gene editing tool concentration, etc. Current technological means often find it difficult to confirm all off-target mutations produced in gene editing. In other words, it may be difficult for us to fully confirm whether Lulu and Nana encountered this problem. Off-target: The current level of gene editing technology cannot guarantee a 100% success rate and specificity. There is a certain probability of "accidentally injuring" other genes while modifying the target gene, which may lead to serious consequences. Furthermore, whether the expected effect of HIV immunity can be achieved is also a big question mark. The theoretical basis for preventing or treating AIDS by editing the CCR5 gene comes from the famous "Berlin patient" and studies of the North European CCR5 gene mutation population. What is the Berlin patient: The first and currently the only AIDS patient in the world who has been cured. He had both AIDS and leukemia, and after receiving leukemia bone marrow transplant treatment in Berlin, Germany, his AIDS was miraculously cured. Scientists found that the bone marrow donor from Northern Europe had a congenital CCR5 gene mutation. What is the CCR5 gene in Northern Europeans: About 10% of the Northern European population naturally lacks the CCR5 gene. Many studies have shown that this deficiency is significantly associated with their immunity to AIDS. It must be noted that the CCR5 gene deficiency produced in the laboratory is different from the CCR5 gene mutation naturally occurring in nature. Although the experimental details and data are not yet available, based on research experience, it is considered technically difficult to ensure that the genotype they edit is exactly the same as the naturally occurring one. Furthermore, the CCR5 gene has been shown to have biological functions, and its deficiency may also increase the risk of other viral infections and even cancer. Lastly, this mutation gene currently mainly appears in the population of Northern and Western Europeans. Introducing the same mutation into the genetic background of Chinese people may produce completely different effects. He did it purely for his own personal reputation. I may be a simple guy but i got this person's reason on why he did this, well he had a point which is reasonable and acceptable. Personally editing anyone's DNA contains a lot of risk and you must have a lot of knowledge and will to do this for the sake of humanity. Well if this was true and the gene surgery was successful, it can be an answer to cure the incurable disease like HIV. Lastly it takes a lot of courage to do it.

This is scientific and ethical misconduct. You should not only lose whatever credentials Universities have awarded you, but you should potentially face serious personal penalties as well. Did you truly provide these parents with Informed Consent? This wasn't research intended to actually benefit the patients. Was that clearly explained to the parents or did you influence their decision making by suggesting that somehow this would help their children? Did you explain all the potential negative impacts of the process? When faced with an "off target" mutation did you seek independent consultation with clinical geneticists to determine it was unlikely to impact the potential child, or did you just make that decision on your own (with all of the bias involved in your decision making) and present that information to the family? There are so many ethical pitfalls here it is truly frightening. Not to mention your clear desire for publicity and attention.
