The Active Compression, or O'Brien's Test, is one of the earliest and most commonly performed special tests for SLAP tears. It seems like every doctor in the world does this test, and many make decisions based on this test exclusively.
There's only one big problem, I don't think it's very effective...
The original research study showed amazing results, but more recent studies have nowhere near the same results. To me, the test puts the shoulder in a very disadvantageous position, so anyone with shoulder pain (especially if from the cuff) is going to have a positive test.
So use this test with a combination of others. Used alone, I'm not sure it's very valuable.

Great content, always keeping me thinking. In my mind, the shoulder fully externally rotated is too similar to a speed's test. Do you think that is a legitimate concern? Please let me know your thoughts.

yeah good call, maybe. Biceps and cuff really stressed.

what if the result is the opposite?? no pain in the first part of the test, a lot of pain in the second part? the pain is felt near de LHB tendon, in front of the shoulder

Still need to likely do more tests to completely figure out, but would look at biceps and AC joint area.
