I don't agree that it was his mistake. It was just an unfortunate outcome of pregnancy, she was probably dealing with postpartum blues/ depression/ psychosis along with the sad news. I feel that the only mistake was he didn't focus on the road while driving and looked at her sadly when she keeps saying "I shouldn't have, it shouldn't have....."
