Those lows are all him. He can do those live with and without mic. He even has a video where he shows that it's real by doing it on his couch and you can gradually hear his breaths deepen in sound

He has actually admitted that it's scarification, cause when it was done, he posted on his insta that maybe it was too deep. He also admitted it in comments on this platform cause there was rumors going around that it was anything from a car/bike accident through to surgery to put a metal plate in his face because of the accident.
