The only thing I don't understand is the initial grouping. Do you ALWAYS organize by OR groupings or do you go by parenthesis if they exist in the problem? For example how would you write out an equation that doesn't have any OR operators, or one that wraps AND pairs in parenthesis?

The ORs are the delimiter for groups. Each group is independently placed into the kmap

If it doesn't have ORs then it represents an expression with a single term. If you have brackets then it's easiest to expand the brackets first then do the kmap entries
