it seems to be all about the direction you are looking through the wall. I imagine it like a straight line coming from the crosshair in the direction you are looking, and wherever that line intersects with a wall, it makes a portal. so not only do you have to look at the right spot on the wall, but also from the right angle so you are looking at the exit portal through the wall.
