Individuals with sickle cell disease are homozygotes of the sickle gene. This is a genetic mutation that developed on the beta chain of red blood cell hemoglobin, that is protective of malaria, endemic in equatorial Africa, where the mutation is currently known to have originated from. It follows Mendelian inheritance. Sickling will cause pain, anemia, chronic tiredness, and kidney damage, among others.
Sickle cell trait refers to heterozygosity. In trait, the RBC's don't typically sickle in the body, but they will in places that are particularly hypertonic and hypoxic. The foremost example of that type of environment, is the kidney medulla. Hypertonicity and hypoxia are favored in this tissue because of its function in water reabsorption in the Loop of Henle. The descending loop into the inner medulla is permeable to water only. As more water flows out into the medulla, it becomes dilute. In the ascending limb of the Loop of Henle, ions can be pumped out of the filtrate and in to the medulla, maintaining hypertonicity of the tissue, allowing more water to be reabsorbed.
In the vasa recta, the RBC's will sickle into the medulla, causing vaso-occlusion. Meaning the mechanisms of maintaining medulla hypertonicity will be compromised, turning hypoxia to become anoxia (pO2 = 0). This damage will force all filtrate to become urine, so water loss will ensue.
In extreme exercise, this will cause hemoconcentration, hypoxemia, worsening lactic acidosis, and increased sickling.

Without being too graphic, I’ve had discolouration there with no (other) symptoms for the next six hours. I went on a work trip to another city, and found myself in horrible agony when I sat down for lunch later that day. I started a track session with a somewhat sore foot about nine years ago, and decided to keep going. Halfway through my second 400m sprint I suffered a stress fracture and ended up in a boot for the next two months. Sometimes it isn’t always clear that there’s something very wrong, and you go ahead with what was planned.

I never knew when I went through Bootcamp in the navy why people with sickle cell trait had to wear a flag belt during our physical training sessions, but now I understand it was to make sure they got treated properly. Glad to hear they cared about people's health.

There was a girl in my boot camp division with it who collapsed on the track in freedom hall and died and it was found she had sickle cell. This was before they started looking into it. Lawsuits from the family followed, and a few months later, all the new guidelines for sickle cell were put in place. However, the navy had to be threatened with multiple lawsuits first before they would do anything about it. The RDC’s cared. The people who were there, who saw it, cared. Big Navy, though? Hmm.

(This is EXTREMELY RARE and I still believe this man's older age contributed to this unfortunate outcome.) - A man who accidently injected himself with cow antibiotic in a freak accident and died from cardiac arrest. - A theoretical case of a man eating chicken cooked in NyQuil and drinking the remaining liquid and dying from liver failure brought on by a massive Acetaminophen overdose. - A man who got addicted to cough syrup and despite recovering, the addiction eventually killed him. - A woman who was a solder in the Middle East, died almost 20 years after exposure to chemicals that were carcinogens. - A farmer who accidently drank diquat herbicide and despite his and doctor's best efforts, he died from a heart attack in the ICU. - The experience of a women who drank 1 liter of soy sauce for a sham colon cleanse and suffered from acute hypernatremia (massive sodium overdose), which led to her becoming quadriplegic and almost unable to live (definitely unable to live without support). - A man who was murdered via thalium poisoning by his wife. I have sickle cell trait and was told by doctors that the only things that would trigger an episode was diving too deep or flying too high. Found out the hard way that extreme heat and overworking can also cause episodes. In the past there weren't a lot of studies being done and things have usually "righted" themselves by the time you get to medical help. Most doctors have no clue what's going on, unless you happen to know and are able to tell them when you are being examined.

@silviazolam 4 года назад
Hematuria (presence of blood in the urine), in general, is not an emergency. It can be caused by a lot of conditions ranging from a ordinary UTI (urinary trait infection) to kidney necrosis and cancer. What I mean is you shouldn't go to the ER just because your urine is red: it's not that alone that defines a medical emergency situation, but the presence of other symptoms indicating a possibly life-threatening situation, like traumas, acute abdominal/pelvic pain, very high fever, profuse vomiting, consciousness alterations and so on... Also, if it happens that you suddenly are unable to urinate anymore, you should get visited as soon as possible because kidneys can quickly get damaged in that situation. Remember that the emergency room is meant for emergency situation: don't overcrowd it if not needed. That is a huge problem we have in Italy and I can assure you that the consequences are devastating for real urgent patients... Furthermore, red urine doesn't always contain blood. It can be caused by some foods and/or some drugs that may color your urine without the presence of blood. There are also other conditions where you have only hemoglobin or mioglobin (and not blood cells) that color your urine, but I don't want this comment to be excessively long. What you should do if your urine turns red and you don't feel so bad that you'd need to go to the ER is: have a urine test and visit your family doctor within two days. Just keep calm and start from there

Hardly. These things are "signs" in retrospect - or do you think all college students have advanced medical training? The guy should have known red urine is a bad sign, but I expect like most guys he though it would correct itself. If the person complaining doesn't seem concerned, why should you be?
@cavalieroutdoors6036
@cavalieroutdoors6036 4 года назад
@@Cheepchipsable The reason you should be worried is because his urine was red. Unless you bruised your kidney via light sparring (or something similar), and you know that's exactly why you're pissing red, that isn't something you just brush off in the hopes it fixes itself. And even under those conditions, you should probably go get checked out because there's always the chance it won't heal like a normal surface level bruise in a week or so. I don't expect college students to realize this of course. But I do expect coaches or other athletic trainers to realize it if they catch wind of it.

well when you work out your muscles develop little tears (that's why you feel sore), which your body then regenerates with protein. but if you don't give your body time to regenerate you are basically just tearing your muscles more and more. that's why it's advised to have a break day between workouts. he worked out to the max three times a day, so you can imagine he was just straight up destroying his muscles and nothing else

And he’d already told mates that he’d seen blood in his urine. Is it an American thing where it’s like “man up we are America we are Trumps children and we rep till we drop or till our bodies pop” because I had an American friend Brad Epstein and he always used to say after crushing a can on his head “I am American im all for greed and I never read, we want war don’t care what for, I love meat you like to be neat, yeah yeah I am an American” so I wonder if it was an ego thing about being a learning new country? Like small man syndrome? My friend Brad died eating a burger in ‘American worlds largest burger’. It’s sad because if KG was somewhere else they would of known that blood in urine, not being able to see, slurred words and falling while repping was not common. Or maybe they thought that is natural after 200 reps because they’d never seen it before? Makes you think 🤔

If you're in good health and want to do 500 squats, though, for the love of God and all that is holy, work your way up to 500. If you just go from 0 to 60 like that, even outside of his sickle cell trait, you're liable to tear a muscle or even worse by over exerting yourself like that. And in 20 minutes? That's an obscene level of over exertion. Given his condition, it's really no wonder that this all transpired. To give you perspective, he was completing a squat, on average, every 2.4 seconds for 20 minutes strait. My legs are hurting just thinking about it.

the deep breath made his lungs hyper-stretch, which shouldn't do that. Upon further inspection found that he had a rare mutation that leads to his lungs being way too small. but the brain should know that and commit suicide right after birth, but it didn't, because of another syndrome called total stupidity. the brain was so stupid that it didn't know what the body's condition in. but this should also affect his communication skills and everything else, but again it didn't. because he also have a mutation called BBC or buttock-brain cancer, which a tumor formed in his to buttcheeks but then became the brain. a million more syndromes and mutations later: ... he then died because the BBC was out of control and his entire body was being hijacked by the tumour brain cells. yes when he died his entire body's brain. so whatever you do, don't take a deep breath. take moderately deep breaths and you'll be ok.

Considering he became depressed from a break up then fell behind his peers, it sounds like he tried to overcompensate to get back where he was, possibly with a mentality similar to those who do illicit drugs or alcohol, likely thinking that because it's exercise, it was healthy. This like most of these videos is completely preventable with a modicum of common sense. We are taught to do things a certain way for no good reason...but that's because you need to be a Dr and a pretty good one to boot, to actually understand the reason for everything "common sense" is telling you to do or not do... so confusing. Now pass the expired gas station sushi and lava lamp juice chaser! Common sense says 500 reps 3 workouts per day = over training. You can literally work yourself to death. This is so tragic. I emphasize very much because my dad was told he will have to watch out for renal failure in his 20s, and stuck to it until he was in his 50s after bouts of gout and finally had his kidney transplant. He still doesn't know what caused it in the first place. I myself was like this person when I first entered university. I was just trying my hardest to do my best academically for a whole year. But, after going through a lot of stress and being overweight with not so good of a diet, I was diagnosed with Type 2 diabetes. There was no family history of diabetes as far as I know. The only explanation that my doctors could come up with was I was probably born with smaller pancreas because I'm of East Asian ethinicity, and that the undue stress and insomnia probably was what broke the scale. I still remember the feelings of guilt, thinking that I should have known better and that it was so unfair that I will have to live with the condition for the rest of my life because I was genetically predisposed. Because they aren't sure what caused my dad's renal failure in the first place, I have to be extra careful about my diet and any medications I am taking so my kidney will be damaged. I feel sad about all this because nobody teaches you until it's too late to prevent the problems. Then, you are left with the fall outs and damage control. It's also psychologocally damaging in the long run because you slowy become less motivated in the fighting the long fight as evidenced by my eventual depression. Thankfully it is being managed to the best of my ability.

I know of rhabdomyolysis as something Crossfit obsessives get with alarming regularity and are horrifyingly blase about. Even if you don't have sickle cell trait, this can happen to your body; and even surviving it still means it has wrecked your muscles. Do not overestimate your body's capacity to exert itself. Rest in peace, KG.

Right? I had a feeling this case would be related to that when I saw the title. A lot of old school Crossfitters talk/ed about having “rhabdo” as some kind of funny right of passage (complete with an awful clown mascot with his kidneys falling out). Hopefully people are more aware of the dangers now. This video made me so sad, rest in peace KG x

This kind of mindset reminds me of old ED circles I used to frequent ten years ago where you were only cool if you had been admitted to the ER at least once, as if it was some kind of flex. I remember flexing that I was entering the "danger zone" of weight as well. Excessive anything is never the answer, specially at the cost of one's own body.

It's how I met him actually. Back on twitch, he came into a stream once and we hung out on discord. He's a super chill dude. Glad to see his channel is still going strong

And the videos are still up because Bernard doesn't hide his past or from it. (And why would he? He's a ripped ex power lifter with degrees in chemical engineering and medicine!)

His name is Bernard! I have never seen someone match that name so perfectly. I love this fun fact. Bernard is so formal and fancy, I gladly expect eloquent case studies from a Bernard

Yes another fatal end caused by an emotional collapse. That's why having a well trained mind with a good amount of self knowledge and strong spirit is vital and here is proof it can actually save your life.
