An update on dissolving heart stents

Dissolving heart stents are an exciting prospect in the management of patients requiring heart stents to open a blockage within the arteries of the heart. At present, these devices are not standard of care and therefore most patients who need a stent will have a permanent type device inserted. As technology improves, the tools available to manage blockages will expand and there may come a time where these dissolving stents offer the same if not better outcomes compared to traditional permanent stents. Join Professor Barlis as he discusses these devices and the current and future prospects with them. One thing no one seems to talk about or warn potential stent recipients of is an autoimmune reaction to the material; my friend just had three (non-dissolving) stents put in and within a few weeks developed arthritis so bad he can barely use his hands-not sure the trade-off was worth it, and someone should have advised him that this was a possible outcome. I'm appalled at how far behind the times American medicine is (I had to have cervical vertebral fusion a few years back, but the optimal choice-flexible rubber replacement discs, which are routinely used in Europe-was not legal in the States), and I hope your team and others make a breakthrough with the dissolving stent technology soon!

I love the concept and asked my cardiologist about these and he told me all of the tests failed and this project was essentially abandoned.

Same here, said the trials did not prove a benefit yet. He said it would be many years before it becomes approved but he feel that it will eventually get approved.

Professor, I really appreciate the information you are sharing in all your videos, it demystifies these great life saving devices and the procedures taken to place them. I'm sure a lot was explained to me at the time I required the procedures but I wasn't in much of a condition to absorb the information. Just felt blessed to be able to be alive.

I am 58 years old with a family history of Heart disease and I just had 2 stents put in the same artery after experiencing some angina. Because technology and medical science is always developing I wish that I would have had been given the option of a dissolving stent. Especially at my age where I am still very active.

Thanks for sharing Mike! Results with conventional stents are still excellent even after many many years. Key is we keep a check on our risk factors (not much we can do about a family history) but so long as we keep blood pressure, blood sugar and cholesterol levels on the low side outcomes are excellent.

@@HeartMatters I guess my main concern is how long do the stents last? Dissolving stents seems to cure the problem of what to do when a stent fails. From a layman's POV you could just go back in and put another stent in.

From what I’ve read, the failure and complications of stents is underreported.

Peter i have question ,, 4 months ago i had a heart attack doctor put a drug eluting stent in me if in 4 or 5 years from now if magnesium sound promising could l have mine replaced with a magnesium

Great question. In short, once a permanent stent is placed it cannot be removed and stays for life. At present, if in the rare event something was to happen within old stents, we would not recommend using the dissolving stents as we dont know much about the long term results in using these stents to treat within old stents. There is however a technique using a drug-coated balloon that can be excellent in treating within old stents but without having to use more metal/new stent inside. We will focus on this in a future video too.

Is it true that another problem with dissolving stents is that this process of dissolving might cause micro-fragments of stent material to dislodge, circulate in the blood and getting stuck and/or doing fatal damage in other vital organs? Which brings another question: are these stents healing like metal ones, i.e. being slowly covered in new endothelial tissue? And if that is the case, does the dissolving process starts before or after they are covered in new tissue?

It seems to me that until there is a technique to remove the plaque from the affected artery, that the matter of stents, stent replacement etc. will be an ongoing issue. How can the plaque be removed and this requires a determination as to how it got into the blood vessel in the first place.

Excellent point Stephen. You are correct, stents are 'reactive' to treating a blockage that is already there. There is considerable research on trying to target plaques that are not causing major blockages now to help prevent them from growing and causing complications long-term. This is all exciting research for the future.

I had plaque removed with surgery, all went well.... 6 months later , just finding out that I need a stent placed. Who knew? that scar tissue can block your artery...( 65% blockage)

In autopsy 29% of metal stents were broken, not a small number.

Yes David, these devices are prone to metal fatigue, we have had a video on stent fracture posted you may find relevant

@@HeartMatters Thanks for reply, I’m not finding much detail in the peer reviewed literature on the mechanics of this problem, what’s driving fractures, fatigue? Prolonged high HRs? High MAP? LV hypertrophy?

That is exactly why so much effort is continuing on the novel dissolving stents meaning that nothing permanent is left behind and therefore these anti-clot treatments do not need to be continued long-term. Interesting to see how we can advance science and technology in this field. Thanks for your comment!

I watched another video where this subject was discussed. The solution was bypass surgery.

Bypass is a very invasive procedure correct? Stents are minimally invasive but taking more than aspirin for life doesn't appeal to me. When you ask they say Plavix or similar for at least 1 year, maybe 3 years, and maybe for the rest of your life.@@e.miller8943
