This was such a cool video! As soon as you stepped into the Pain Management room, I was both surprised and very familiar with that setting. I dealt with a few years of chronic pain in my shoulder, elbow, and wrist after a serious accident. I had a few exploratory examinations and injections done in an office just like that, as well as imaging for nerve damage and fittings for custom wrist/elbow braces. I had no idea that fell under anesthesiology!

Hello!! I’ve just graduated pediatric residency (5 years in Italy). I’ve spent last 3 years in neonatal and pediatric ICU and I’d like to apply for pediatric critical care medicine fellowship! I’ve spent all 2022 in my local PICU (Verona - the city of Romeo and Juliet 😂) and I just fell in love! I love the precision needed in procedures and the variety of disease and patient! Oh, those nerve blocks... I've had more ilioinguinal blocks and ablations than I can count. I've become quite familiar with the various machines they use. Glad I'm beyond that.

Hi Max. Great video. I would think you’d be great at pediatric anesthesia and even on the acute pain service. I’m 50yo C5-6 Quadriplegic who’s currently in a hospice center getting my medication managed better before continuing my journey at home or another facility. Anesthesia has been an integral part of my care for thirty plus years, allowing me to function and live independently post injury. But in today’s climate of the ‘opioid crisis’ it has become next to impossible to fill my medications at the pharmacy. Doctors are scared of losing their license and pharmacies scared of DEA intervention. Maybe some time you can touch on those things...thanks again

Just wanted to thank you again - I was in the cath lab a couple of weeks ago for a cardioversion (unsuccessful unfortunately so I'm looking at an ablation) and I wasn't frightened at all by all of the hardware in the room. I thought it was really interesting that there was one huge table in the middle of the room was this huge platform (I guess a bed) and at the foot was one of the largest monitor screens I have ever seen hanging from the ceiling- had to be a good 6'-8' across. I wonder if on off hours they use it to watch football/soccer? :-) Anyway, thanks.

Cool that you ended up being interested in peds. I remember you beginning that rotation and it seems like it was your favorite! As is mine. Not sure if I'll go cardiac peds or not. Too much stress and too much training

Hi Dr Max! Wow, a lot of information so fast. Glad to see you back. You will breeze through the pediatric anesthesia thing. You will be great with children. Thanks for another great video. I’m going to watch it again! I have had many of those steroid epidurals over the past 20 years. I don’t mind them at all. I only hate the questions you have to answer 50 times like “what’s your name”. “What’s your birthday” “what are we doing for you today” “are those your real teeth” “do you have any jewelry on” “are you allergic to any medication” “did you eat today” etc etc etc lol Happy Holidays to you and your family! Medication is hard to get for sick people…my theory is they are saving it up for Hollywood, Athletes, and Aristocrats! I’m not in any of those categories. I am only a73 year old disabled man. What do I need with controlled substances for pain!

I love when you talk about pediatric anesthesia because I'm an adult but have to have pediatric anesthesiologist work on my case when I have surgeries. I have to go to a hospital that offers the pediatric anesthesia and doctors

With my first csection I felt it when the doctor tested me for feeling and I told him but the anestesiologist told him that I was ready and she told him that I couldn't be feeling it. I said I don't knwo what I should or shouldn't feel but I felt that but the doctor after arguing with the anestegiologist he started and then when there was no doubt that I was feeling it he ordered the anestegiolist to give me more meds and she said that she couldn't until my daughter was born, so the doctor said he'd get her out as fast as he could. They waited what seemed like an eternity for me to clearly verbally acknowledge that I saw her before knocking me out. When i had my 2nd csection that anestegiologist was no longer working at that hospital and I told the anestegiolist that was there for my son's birth that if that happened again to just end the pain as soon as she could. She told me what to expect to feel and what was happening in the surgery and what I might feel as it happened, which was very reassuring. My 2nd csection was painfree and it was great to feel that the anestegioloist was there to help, where for the first I felt like a slab of meat. The anestegioloist who was there for my 2nd csection was also there for my gallbladder surgery and while I only saw her in the prep, it was nice to know that I was going in with a familiar and safe face.

I know you're a resident so it may be hard for you to answer, but what have your perceptions been of real-life lifestyles of anesthesia vs surgery? I'm an M3 wrestling between anesthesia or surgical subspecialty, eventual lifestyle is important to me. Some people say there is no difference between the two, others will say anesthesia has much more flexibility and is less hours overall. I watched your video on reasons for choosing anesthesia, but curious how much (if at all) this factored into your decision. Thanks!

What's the workup and risk evaluation for patients with things like adrenal insufficiency and diastolic dysfunction? Do these present obstacles for anesthesia? What if the patient has suspected mitochondrial myopathy? Are they at increased risk for things like anoxia or spontaneous cardiac arrest while under? If a patient had one or more of these but potentially suspected more how could they communicate this information with their doctors to get evaluated effectively and reduce the risk for a required surgery?

I was under the impression that all these subspecialties besides critical care and pain is not required to do anesthesia in those situations? I have heard of non cardiac fellow anesthesiologists do anesthesia for sick cardiac patients (for example). Any clarifications on this? Thanks for the content man.

I have a question about getting an epidural during labor. I have 3 kids..with each birth I had an epidural. The first two worked fine and getting the epidural was fine. The third time was terrible. The anesthesiologist stabbed me with the needle like ten times. It was so painful. He told me he was having trouble placing the needle because I have scoliosis. The other two times I had an epidural they did not have any trouble with this. Put needle in only once. What do you think of this? Also the third time the epidural did not work…only numbed my legs.

Thanks for another great video! If you do a critical care fellowship, could you still practice general anesthesia as well as NICU for example? Or would that require additional fellowship to work in the NICU? Thanks!

Hi Emily, if you do critical care then yes you can still practice general anesthesia. I actually don't know for certain whether a critical care trained anesthesiologist can attending in the NICU -- I thiiiink a Peds anesthesia fellowship may be required in addition but I really don't know.

Generally speaking, subspecialty-trained anesthesiologists make slightly more per year than general anesthesiologists, but it evens out over time because the general anesthesiologist makes a full attending salary during the year that a subspecialist is doing fellowship. The only subspecialty that consistently makes more than average is pain management. Hope that helps!

Question. U speak of subspecialtiees. 1 being neuro. My husband had 2 brain surgeries 14 hrs apart.. so would that require a anesthesiologist of neuro or is there ones who just do brain surgeries. 2nd question is like where do u sit or stand as thhe surgeons would be in your space ? 🤔

Hi Sherry! General anesthesiologists are typically able to care for patients undergoing neurosurgery, so no, a subspecialty-trained neuroanesthesiologist isn't required. Also, for neurosurgery, usually the operating table is turned 180 degrees away from us after we induce anesthesia. Then we turn it back around once the surgery is done. Great question!

Dental anesthesiology is a subspecialty of dentistry, not anesthesiology. That said, anesthesiologists do provide anesthesia for dental cases that occur in operating rooms.

If your doing teaching .... MAKE SURE YOU TEACH YOUR STUDENTS HOW TO ADMINISTER THE ANISTESIA TO PATIENTS WHO ARE ALLERGIC TO THE AFTER EFFECTS OF THE ANISTESIA. ... Five surgeries, only ONE knew how to administer the anesthesia. violently sick .... Poor training.
