It was a way for them to keep from spreading their illness to others if they had to go to work, run errands, etc. Then in 2003 a company took the basic onetime use surgical masks and designed reusable ones with breathable mesh fabric. These were created and promoted for people who suffer from hay fever and other allergens. It allowed allergy suffers to protect themselves outside during allergy season. Then around the time the H1N1 and influenza outbreaks started Japanese people actually started wearing them everywhere in an effort to keep themselves from getting sick. Imagine being cramped in a subway pressed up against hundreds of people while commuting to work all the while a serious flu epidemic is killing people.. Yeah I’d wear a mask too. Today tho, if you’re not wearing one to keep from getting sick and/or spreading sickness you’re wearing it just like he is - for fashion purposes. Famous designers in Japan and other places now create masks with intricate designs and logos helping to turn something with a negative connotation about illness into into something stylish. So yeah the use of surgical masks may have started as a way for sick people to go about their day to day lives without fear of spreading their illness to others but they now have a plethora of uses. Imagine it like how people wake up every morning and put on their glasses, or their earrings, even watches - the uses are varied and endless.
