Hi I have a question : Patient with history of stroke 7 years ago , he was previously on xarelto (then stopped cuz he switched providers who was giving him samples ) - then he was started at some point on Coumadin -- then he could not afford and was switched to eliquis .... He completely stopped taking anticoagulants 6-7 months ago -- because he can’t afford them -- There are some things he has difficulty recalling so ... Still waiting on medical records at this point What would you do ? Would you restart warfarin ? Since it is the cheapest but the one medication that requires INR checks ?

Definitely check out the cheat sheet and it goes over it-- It's the percentage of the total dose. For example, if the daily dose is 10mg, decreasing by 10% is decreasing the dose by 1mg. So the new daily dose would be 9mg. Does that make sense?
I don't see much heparin use in primary care, but primarily it's LMWHs though for patients with a VTE after hospital discharge (DVT, PE, etc). Prescriber's letter is a great pharmacology resource to learn more!
